【摘要】 目的探讨声学造影结合常规超声检查在诊断坏疽性胆囊炎中的价值。方法回顾性分析经手术病理证实的57例胆石性胆囊炎病例(坏疽性胆囊炎15例、化脓性胆囊炎42例)的术前常规超声及造影检查特征。结果胆囊壁造影剂回声强度减弱、囊壁完整性欠佳、胆囊窝积液等征象在坏疽性胆囊炎与化脓性胆囊炎组间存在统计学差异(P<0.05)。其中以胆囊壁造影剂回声强度减弱为参数鉴别坏疽性胆囊炎和化脓性胆囊炎的灵敏度与准确度分别为86.7%、91.2%,高于其他超声检查指标(P<0.05)。结论声学造影可以通过显示胆囊壁的低灌注状态及囊壁的连续性中断等声像特征,与常规超声相结合,为协助临床早期诊断坏疽性胆囊炎提供重要信息。更多还原

【Abstract】 Objective To explore the value of contrast-enhanced ultrasonography(CEUS) together with routine ultrasound examination in the diagnosis of gangrenous cholecystitis.Methods 57 Gallstone cholecystitis cases with gangrenous cholecystis and purulent cholecystitis confirmed by surgery and pathology were concluded.The characteristics of routine ultrasonography and CEUS before operation were retrospectively analyzed.Results Three features including hypoenhanced gallbladder wall,discrete wall and pericholecystic fluid were found valuable in distinguish gangrenous cholecystitis from purulent cholecystitis with Statistical differences(P<0.05).The sensitivity and accuracy value of hypoenhanced wall in differentiating gangrenous cholecystitis and purulent cholecystitis were 86.7%and91.2%.highest in all the features(P<0.05).Conclusions CEUS is of great help in the the diagnosis of gangrenous cholecystis through some tipical features such as hypoenhanced and discrete of gallbladder wall.更多还原
