高效抗病毒治疗后病毒学失败的儿童艾滋病29例HIV-1基因突变和耐药分析

【摘要】 目的了解河北省参加抗病毒治疗的儿童艾滋病患者病毒学失败后HIV-1基因特征,并分析其耐药性。方法采集河北省辖区内参加高效抗病毒治疗(HAART)后的68例儿童艾滋病患者静脉血液,测定其病毒载量,对病毒载量≥1 000拷贝/m L者进行HIV-1基因型检测。结果 68例患儿童中,29例出现病毒学失败,其中15例(22.06%)出现耐药基因突变,而且该15例患儿未出现仅针对PIs的耐药性突变,余患儿中有1例出现PIs,NRTIs和NNRTIs多重耐药。逆转录酶抑制剂(NRTIs和NNRTIs)双重耐药率为27.59%(8/29)。高度耐NVP的频率最高,占51.72%(15/29)。结论 HIV-1基因突变诱发耐药性是导致儿童艾滋病患者治疗失败的主要原因。开始抗病毒治疗前适时进行HIV-1基因型检测可为制定最佳治疗方案奠定基础。

【Abstract】 Objective To assess the HIV-1 genotypic characteristics among children with virological failure after highly active anti- retroviral therapy( HAART) in Hebei. Methods Plasma samples were collected from 68 children who accepted HAART,and their viral load were detected. Among children with viral load ≥1 000 copies / m L,the detection of HIV-1 genetype was carried out. Results Among 68 children with AIDS,29 cases occured virological failure. 15(22. 06%) cases with virological failure exhibited the resistance to antiretroviral drugs,butno drug- resistance to PIs apart from the multiple resistance to PIs,NRTIs and NNRTIs(1 case). The rate of dual resistance to NRTIs and NNRTIs was 27. 59%(8 /29). The rate of high drug- resistance to NVP was up to 51.72%(15 /29). Conclusion The main factor which resulted in virological failure was HIV-1 gene mutation which caused drug resistance. The detection of HIV drug-resistance ahead of HAART was useful so that we can make better plan of ART.
