棘突椎板复合体原位回植椎管成形术在椎管肿瘤手术中的应用

【摘要】 目的探讨棘突椎板复合体原位回植椎管成形术在椎管肿瘤术中的意义。方法分析总结2010年1月至2013年6月间共18例椎管内肿瘤患者术中采用铣刀卸下椎板,处理完椎管内肿瘤后再将棘突椎板复合体原位回植,予钛条钛钉固定,恢复椎管正常解剖结构。结果所有病例未出现因铣刀卸下椎板,棘突椎板复合体原位回植固定而导致硬脊膜破损、脊髓和脊神经根损伤等并发症。随访3~18个月,未见回植组织移位、塌陷、畸形愈合及椎管狭窄症。结论椎管内肿瘤术中棘突椎板复合体原位回植椎管成形,钛条钛钉固定的方法简便、实用,有利于恢复脊柱的正常解剖结构及稳定性。

【Abstract】 Objective To investigate the spinous process and vertebral plate complex in situ replantation laminoplasty in intraspinal tumor clinical significance. Methods From January to 2013 of 2010 are analyzed in June a total of 18patients with intraspinal tumor treated by cutter removed lamina, after treatment of intraspinal tumors after thespinous process and vertebral plate complex in situ replantation with titanium, titanium screw fixation, restore thenormal anatomic structure of spinal canal. Results All patients does not appear due to milling cutter removed lamina,spinous process and vertebral plate complex in situ replantation fixed and lead to breakage, epidural spinal cord and spinal nerve root injury and other complications. Patients were followed up for 3 ~ 18 months, no implant tissue displacement, collapse, malunion and spinal stenosis. Conclusions Intraspinal tumors spinous process and vertebral plate complex in situ replantation laminoplasty, titanium titanium screw fixation method is simple, practical, is conducive to the normal anatomical structure and restoration of spinal stability.
