【摘要】 目的观察布托啡咯复合丙泊酚用于无痛胃镜检查的麻醉效果。方法选择ASAⅠ~Ⅱ级无痛胃镜检查患者120例,分成三组各40例。A组仅给予丙泊酚;B组先给予布托啡咯10μg/kg静脉注射,再静脉注射丙泊酚;C组先给予芬太尼0.5μg/kg静脉注射,再静脉注射丙泊酚。三组患者均按照丙泊酚1.5~2.5 mg/kg静脉注射,待患者入睡后再行胃镜检查,必要时追加丙泊酚。结果三组麻醉效果比较差异无统计学意义(P>0.05),B、C组VRS评分0分率高于A组,差异有统计学意义(P<0.05);麻醉后三组平均动脉压均明显降低(P<0.05),但组间差异无统计学意义(P>0.05);用药后C组心率明显减缓,与A、B组比较差异有统计学意义(P<0.05);B组呼吸抑制发生率低于C组,差异有统计学意义(P<0.05)。结论丙泊酚复合布托啡咯或芬太尼用于无痛胃镜,麻醉效果优于单独使用丙泊酚。丙泊酚复合布托啡咯不良反应少,安全方便。更多还原

【Abstract】 Objective To observe the anesthesia effect of butorphanol combined with propofol in painless gastroscopy.Methods One hundred twenty ASA Ⅰ ~ Ⅱ grade patients,who were ready to accept painless gastroscopy,were randomly divided into three groups,40 in each group.The group A was only intravenously injected with propofol.The group B was first intravenously injected with1μg/kg of butorphanol and then propofol.The group C was first intravenously injected with 1μg/kg of fentanyl,and then propofol.The patients went to sleep before gastroscopy.If it was necessary,additional propofol will be re-injected.Results There was no significant difference in anesthesia effect among the three groups.VRS scores in the groups B and C were significantly higher than that in the group A(P <0.05).MAP was dramatically decreased in all the three groups(P <0.05) but no significant difference was found among the groups.After injection of drugs,HR in the group C were significantly decreased when compared to other two groups(P <0.05).The incidence of respiratory inhibition in the group B was lower than that in the group C(P <0.05).Conclusion Combination of propofol with butorphanol or fentanyl has better anesthesia effect than propofol alone in painless gastroscopic examination.Combination of propofol with butorphanol has fewer adverse reactions and it is safe and convenient.更多还原
