帕金森病膀胱尿道功能障碍尿动力学分析
Urodynamic Analysis in Vesicourethral Dysfunction of Parkinson’s Disease
【摘要】 目的研究帕金森病膀胱尿道功能障碍患者尿动力学异常,并与前列腺增生症排尿异常进行比较。方法帕金森病膀胱尿道功能障碍患者30例,前列腺增生患者64例。患者均行尿动力学检查,分析膀胱尿道功能。结果帕金森病膀胱尿道功能障碍主要尿动力学异常表现为不稳定膀胱共21例(69.4%),逼尿肌收缩功能减弱5例(16.7%),逼尿肌及尿道功能正常者4例(12.5%),逼尿肌尿道括约肌不协调2例(6.25%),低顺应性膀胱10例(34.4%),膀胱出口梗阻13例(43.3%)。膀胱最初容量为80.4±24.9ml,膀胱最大容量203.4±61.7ml,最大尿流率为10.2±6.1ml/s,最大尿流率时逼尿肌压力为52.9±36.2cmH2O,与前列腺增生组在膀胱初感容量、膀胱出口梗阻有显著差异。结论帕金森病膀胱尿道功能障碍患者尿动力学异常主要是不稳定膀胱,部分患者出现膀胱容量减少、逼尿肌收缩功能减弱。尿动力学检测对治疗帕金森病膀胱尿道功能障碍有重要的指导意义。
【Abstract】 Objective To analyze urodynamic findings of parkinson’s disease( PD),and compare with those of benign prostatic hyperplasia( BPH). Methods Urodynamics were performed in 30 patients after parkinson’ s disease,64 males with BPH. Results The studies in 30 PD revealed various findings,unstable bladder in 21( 69. 4%),weakened detrusor contraction in 5( 16. 7%),normal function in 4( 12. 5%),detrusor- sphincter dyssynergia in 2( 6. 25%),low compliance of bladder in 10( 34. 4%),and bladder outlet obstruction in 13( 43. 3%). The capacity of initial bladder sensation was 80. 4 ± 24. 9ml,maximum bladder capacity was 203. 4 ± 61. 7ml, maximum flow rate was 10. 2 ± 6. 1ml / s,detrusor pressure at maximal flow rate( PdetQmax) was 52. 9 ± 36. 2cmH 2 O. Conclusion The urodynamic abnormalities were mainly detrusor hyperreflexi in vesicourethual dysfunction of Parkinson’s disease. Partly were the unstable bladder,hyporeflexia or areflexia. Urodnamic examination will provide evidences for the proper treatment for these patients.
