河北省甲型H1N1流感裂解疫苗大规模接种前后格林-巴利综合征发病流行病学分析

【摘要】 目的了解甲型H1N1流感疫苗接种前后格林-巴利综合症(Guillain-Barre Snydrome,GBS)发病的流行病学变化,研究甲型H1N1流感疫苗接种对GBS发病的影响。方法采用前后比较的方法对河北省甲型H1N1流感疫苗大规模接种前(2006-2009年)、接种后(2010-2012年)GBS发病情况进行分析比较。结果河北省15岁以下儿童GBS在甲型H1N1流感疫苗接种前后均为男性多于女性;发病率以6岁以下的学龄前儿童最高,为1.18/10万;其次是6~11岁的小学生,发病率为0.57/10万;1~14岁初中学生发病率最低,为0.39/10万;疫苗接种后,GBS发病率在0.42/10万~0.57/10万间,平均发病率0.49/10万,较疫苗接种前GBS发病率下降54.2%,经比较差异有统计学意义(χ2=88.55,P<0.01);疫苗接种前后,GBS发病的季节构成经比较差异无统计学意义(χ2=2.24,P>0.05)。结论大规模接种甲型H1N1流感疫苗前后,河北省15岁以下儿童GBS发病的流行特征没有明显差异,建议进一步加强河北省15岁以下儿童GBS监测数据资料以及流感疫苗接种数据的收集,为研究GBS发病和流感疫苗的关联性提供参考。

【Abstract】 Objective To understand the epidemiological changes of Guillain- Barre Syndrome( GBS) before and after using H1N1 influenza virus vaccine,study the influence of H1N1 influenza virus vaccine on GBS incidence. Methods Analyzed the incidence of GBS in Hebei province by means of compare before using the H1N1 flu vaccine( 2006- 2009),and after using the H1N1 flu vaccine( 2010- 2012). Results Male is higher than female among children under 15 years old children before and after the H1N1 influenza virus vaccine in Hebei province; the GBS incidence of preschool children under 6 years old was the highest of 1. 18 /100000; the incidence of GBS of school- age children aged between 6- 11 yeas old was the second of 0. 57 /100000; the lowest incidence rate was among middle school student aged 1- 14 years old of 0. 39 /100000;After vaccination,the GBS incidence rate is between 0. 42 /100000- 0. 57 /100000,the average incidence of0. 49 /100000,decreased 54. 2% compared to GBS incidence before vaccination; there is significant statistically difference by comparison( χ2= 88. 55,P < 0. 01); Before and after the H1 NI flu vaccine,there was no significant difference of GBS incidence season constitution ratio by comparison( χ2= 2. 24,P > 0. 05). Conclusions After mass vaccination H1N1 influenza vaccine,there is no significant differences of epidemiological characteristics for GBS incidence among children under 15 years,it is suggested that to strengthen the GBS surveillance resources and the collection of influenza vaccination data further,to provide a reference on research the relevance of GBS incidence and influenza vaccines.
