超声心动图评价酒精性心肌病大鼠心脏结构和功能的研究
Evaluation of Cardiac Structure and Function of Rats with Alcoholic Cardiomyopathy by Echocardiography
【摘要】 目的探讨超声心动图在评价酒精性心肌病大鼠心脏结构和功能中的应用价值。方法选取雄性Wistar大鼠45只,随机分为模型2月组(10只)、模型4月组(12只)、模型6月组(13只)及正常组(10只)。模型组进行酒精灌胃及酒精随意饮用;正常对照组选择相同时间及相同剂量生理盐水灌胃。分别于2个月、4个月、6个月后进行超声心动图检测,并将模型组与正常组超声结果进行对比。检测结束后处死模型组和正常组大鼠,取心脏组织进行病理检测,将超声结果与病理结果进行对比。结果经超声检查,模型2月组与正常组大鼠心脏超声比较未见明显差异。模型4月组与正常组相比,舒张末期左心室后壁厚度(LVPWDd)、舒张末期室间隔厚度(IVSDd)、左室舒张末期内径(LVEDd)、左室质量(LVM)、LVM与体质量之比(LVM/BM)增大,舒张早期心肌运动峰值速度(Em)、Em/Am减低,且均有显著性差异(P<0.05)。模型6月组与正常组相比,LVPWDd、IVSDd减小,LVM、LVM/BM、LVEDd增大,射血分数(EF)、左室短轴缩短率(FS)明显减低,Em、Em/Am减低,且均有显著性差异(P<0.05)。超声测定的LVM与尸检检测结果高度相关(r=0.84,P<0.05)。经病理检查,模型2月组以及正常组大鼠心肌HE染色未见异常,模型4月组及6月组大鼠心肌HE染色分别符合亚临床期和临床期酒精性心肌病的改变。结论超声心动图可对酒精性心肌病大鼠的心脏结构和功能进行动态、定量的评价。
【Abstract】 Objective To explore the application value of echocardiography in evaluating cardiacstructure and function of rats with alcoholic cardiomyopathy(ACM). Methods 45 male Wistar ratswere randomly divided into normal group(n=10) and ACM group(n=35). ACM group were dividedinto 2-month group(n=10), 4-month group(n=12) and 6-month group(n=13). Rats from ACM groupwere gavaged with alcohol, and then let them drink alcohol at will, while rats from normal group weregavaged with saline at the same time with the same dose. Then rats from 2-month group, 4-month groupand 6-month group were examined by echocardiography, and the results were compared with that of normalgroup. All the rats were killed after examination. Heart tissues of those dead rats were took out for pathologicalexamination, and the results of ultrasound examination and pathological examination were compared. Results There was no significant difference between rats from 2-month group and normal group. Comparedwith the normal group, LVPWDd, IVSDd, LVEDd, LVM and LVM/BM of rats from 4-month groupincreased, while Em and Em/Am decreased, the differences were all significant(P<0.05). Comparedwith the normal group, LVPWDd, IVSDd, EF, FS, Em, and Em/Am of rats from 6-month groupdecreased, while LVM, LVM/BM and LVEDd increased, which were of significant difference(P<0.05). The LVM measured by ultrasound was closely related to autopsy results(r=0.84,P<0.05). According to pathological results, the HE staining in 2-month ACM rats and normal rats werenormal, while that of 4-month ACM rats and 6-month ACM rats were respectively in accordancewith the subclinical and clinical period of ACM. Conclusion Echocardiography can be used in thequantitative and dynamical evaluation of the cardiac structure and function of rats with ACM.
