【摘要】 目的探讨单管连续侧孔胸管引流在电视胸腔镜辅助下(VATS)肺叶切除术后应用的可行性。方法 VATS肺叶切术患者92例随机分为两组,术毕分别采用改良单胸管引流(A组,47例)和双胸管引流(B组,45例),比较术后胸管引流相关资料。结果 A组和B组的胸腔引流量、引流管保持时间、自控镇痛按压次数、口服镇痛药给药次数、注射吗啡次数和VAS疼痛评分最大值均无统计学差异(P>0.05)。拔管后,B组有1例因液气胸再次行胸腔穿刺置管引流。结论单管连续侧孔胸腔引流与传统的双管引流效果相仿;但前者置管便捷、创伤小,且花费少,更适合VATS肺叶切除术后应用。更多还原

【Abstract】 Objective To explore the feasibility of postoperative drain with one chest tube with several side holes in the patients underwent pulmonary lobectomy by video-assisted thoracic surgery(VATS).Methods A total of 92 patients underwent lobectomy by VATS was assigned into two groups of A(placing one chest tube,47 cases)and B(placing classical two chest tubes,45 cases).The drain-related indicators were compared.Results There were no significant differences in the amount and duration of drain,maximal value of VAS pain score,the number of patient-controlled analgesia,the times for additional use of oral analgesics and morphine injection between two groups(P>0.05).Reinsertion of chest drain tube was needed in one case of group B because of hydropneumothorax after extubation.Conclusion One chest tube drain has a similar efficacy to two chest tubes,but is with the advantages of easier to place,less trauma and cost,and more suitable for the patients underwent pulmonary lobectomy by VATS.更多还原
