【摘要】 目的探讨老年消化性溃疡(PU)胃镜及临床特点。方法回顾性分析并比较346例老年PU患者(A组)和242例中青年PU患者(B组)的胃镜及临床资料。结果 A、B组胃溃疡和十二指肠球部溃疡发病比例分别为2.12∶1和1∶1.28(P<0.01)。A组溃疡发病部位贲门、胃底、胃体、胃角及残胃溃疡多于B组(P<0.01)。A组有典型临床症状者少于B组,不典型腹痛高于B组(P<0.01)。A组并发症、伴随慢性疾病率、幽门螺杆菌(Hp)感染率以及胃镜活检癌前病变发生率均高于B组(P<0.01)。结论老年PU以胃溃疡常见,临床症状不典型,巨大溃疡多见,并发症及伴随慢性疾病多,Hp感染率高,胃溃疡易癌变,因此治疗的疗程较长,并需要复查胃镜。更多还原

【Abstract】 Objective To investigate the characteristics of peptic ulcer(PU)in gastroscopy and clinical manifestation in 346 elderly patients.Methods The gastroscopy and clinical data of 346elderly(group A)and 242 young and middle-aged(group B)patients with PU were retrospectively analyzed and compared.Results The ratio of gastric ulcer(GU)to duodenal ulcer(DU)in group A was 2.12∶1,which was significantly higher than 1∶1.28 in group B(P<0.01).The occurrences of GU in the cardia,gastric fundus,body,angle and residual gastric ulcer in group A were more than those in group B(P<0.01).The patients with typical clinical symptoms in group A were less than those in group B(P<0.01),but atypical abdominal pain was more than that in group B(P<0.01).Moreover,the complications,accompany diseases,infection rate of Hp and precancerous lesions of in group A were all more than those in group B(P<0.01).Conclusion The PU in the elderly shows the characteristics of GU more common,atypical clinical symptoms,giant ulcer,more complications and comorbidity,high infection rate of Hp and easy canceration,who needs a longer course of therapy and repeated gastroscopy.更多还原
