13C-呼气红外光谱仪在诊断幽门螺杆菌感染中的应用
Application value of 13C-breath-infrared spectrometer in diagnosis of Helicobacter pylori infection
【摘要】 目的探讨应用13 C-呼气红外光谱仪在诊断幽门螺杆菌(Hp)感染中的临床价值。方法分别应用13 C-呼气质谱仪和13 C-呼气红外光谱仪对240例进行13 C-尿素呼气试验(13 C-UBT)患者进行检测,并对检测结果进行比较分析。结果 240例中,应用13 C-呼气质谱仪检测Hp阳性99例,Hp阴性141例,感染率为41.3%。以13 C-呼气质谱仪检测Hp试验结果作为标准对照,13 C-呼气红外光谱仪检测106例Hp阳性患者中,9例假阳性(4‰<DOB值<12‰),134例Hp阴性中,2例假阴性。13 C-呼气红外光谱仪检测Hp的敏感性、特异性、阳性预测值和阴性预测值分别为91.5%、98.5%、98.0%和93.6%,准确性为95.4%。与13 C-呼气质谱仪检测Hp结果相比,差异无统计学意义(χ2=3.27,P>0.05)。结论 13 C-呼气红外光谱仪在13 C-UBT中能够可靠、准确地诊断Hp感染。
【Abstract】 Objective To investigate the clinical value of 13 C-breath-infrared spectrometer in testing Helicobacter pylori(Hp)infection.Methods The 13 C breath tast(13 C-UBT)for detecting Hp infection was performed in 240patients by 13 C breath mass spectrometer(13 CBMS)and 13 C-breath infrared spectrometer(13 CBIS)at same time and the results were compared.Results The positive rate of Hp infection tested by 13 CBMS was 41.3%(99/240).Taking the results of 13 CBMS as the standards,13 CBIS reported Hp positive infection in 106cases,false positive in 9cases,Hp negative in 134cases and false negative in 2cases.The rates of sensibility,specificity,positive prediction and negative prediction were 91.5%,98.5%,98.0% and 93.6%,respectively,with a accuracy rate of 95.4%.The results reported by two tests were not statistically different(χ2=3.27,P>0.05). Conclusion 13 CBIS is a highly accurate method for the diagnosis of Hp infection.
