妊娠糖尿病患者血清脂联素水平与胰岛素抵抗的关系

【摘要】 目的研究妊娠糖尿病患者血清脂联素(APN)水平与胰岛素抵抗的关系,探讨妊娠糖尿病的致病机制。方法检测2011年3月至2012年12月来我院就诊的正常糖耐量(NGT)孕妇50例以及妊娠糖尿病(GDM)患者40例的脂联素水平和空腹胰岛素水平,对两组检测结果进行比较分析。结果 GDM组患者APN水平(7.3±6.2)mg/L明显低于NGT组孕妇APN水平(11.9±3.9)mg/L,HOMA-IR又较NGT组孕妇显著升高(均P<0.01)。结论脂联素不足增加了炎性反应,导致胰岛素抵抗的发生和发展,从而导致妊娠糖尿病的发生。

【Abstract】 Objective To research the serum adiponectin(APN) level and its correlation with insulin resistance in patients with gestational diabetes mellitus(GDM),aiming at investigating the pathogenesis of gestational diabetes. Methods Between March 2011 and December 2012,the serum APN level and fasting insulin were measured in 50 pregnant women with normal glucose tolerance(NGT) and 40 patients with GDM in our hospital,to analyze comparatively the detection results between the two groups. Results The serum APN level(7.3 ± 6.2) mg/L in the patients with GDM was significantly lower than the serum APN level(11.9 ± 3.9) mg/L in the pregnant women with NGT,and HOMA-IR was significantly higher than that in the pregnant women of NGT group(all P <0.01). Conclusion The significantly lower APN level might be an important risk factor to cause insulin resistance in GDM patients. Adiponectin deficiency increases inflammatory response,leading to insulin resistance,thus resulting in gestational diabetes mellitus.
