【摘要】 目的探讨心衰日记模本在慢性心力衰竭患者院外管理中的应用及效果。方法将82例慢性心力衰竭患者随机分为观察组40例和对照组42例,对照组给予常规护理和一般随访治疗,观察组在常规护理基础上,患者在护士的指导下记录心衰日记并给予分区管理。比较2组患者出院后3个月的生活质量,3、6个月后的病死率、再住院率,6个月后的心功能情况。结果观察组患者出院后生活质量、病死率、再住院率、心功能情况均优于对照组,差异具有统计学意义(P<0.05)。结论心衰日记模本的建立有利于提高慢性心力衰竭患者院外管理水平。更多还原

【Abstract】 Objective To investigate the effect of heart failure diary model on external hospital management for chronic heart failure(CHF)patients.Methods A total 82 patients were randomly divided into observer group with 40 patents and control group with 42 patients.The control group contained conventional nursing and regular follow-up treatment,including the CHF conventional nursing,while the observer group contained the heart failure diary model,the CHF patients completed the heart failure diary under the guidance of the nurses.Life quality of the patients 3month after discharge,mortality and readmission rate after 3and 6month,cardiac function after 6month were compared.Results CHF patients in the observer group have better life quality and cardiac function,and less mortality and readmission rate,the difference was statistically significant(P<0.05).Conclusion Heart failure diary is effect in improving the external hospital management for CHF patients.更多还原
