心肌特异性miR-133a与cTnT早期诊断急性心肌梗死的价值

【摘要】 目的 比较血循环中心肌特异性miR-133a与肌钙蛋白T(cTnT)在急性心肌梗死(AMI)早期诊断中的价值。方法 收集67例AMI患者和32名非AMI对照者的血浆标本,通过定量逆转录聚合酶链反应(qRT-PCR)测定血浆中miR-133a水平,同时通过电化学发光法检测血浆cTnT的变化。结果 AMI患者血浆中miR-133a水平较非AMI对照者明显升高(P<0.01),而当这些AMI患者康复出院时miR-133a水平已回落到基线。MiR-133a水平的波动与研究对象的自身特征无相关性(P>0.05)。受试者工作特征曲线(ROC)分析表明,miR-133a在早期诊断AMI方面具有显著的心肌特异性和敏感性,但并不优于cTnT。结论血浆miR-133a可作为AMI早期诊断的标志物,但在诊断价值上并不优于cTnT。

【Abstract】 Objective To evaluate the expression of plasma cardiac-specific miR-133 a in acute myocardial infarction(AMI) and compare their diagnostic values with that of cardiac troponin T(cTnT).Methods Sixtyseven plasma samples obtained from patients with AMI and thirty-two plasma specimens collected from non-AMI volunteers were analyzed in this study.The level of cardiac-specific miR-133 a was detected by quantitative reverse transcription-polymerase chain reaction(qRT-PCR),and the level of plasma cTnT was detected using electrochemiluminescence-based methods on the Elecsys 2010 Immunoassay Analyzer.Results The level of plasma miR-133 a was significantly increased in AMI patients(P<0.01) compared with non-AMI volunteers.The expression of the cardiac-specific miR-133 a in AMI patients decreased to close to the baseline at the time of hospital discharge(P>0.05).There was no correlation between the level of circulating miR-133 a and the clinical characteristics of the study population(P>0.05).Furthermore,receiver operating characteristic(ROC) curve analyses showed that miR-133 a was cardiac-specific and sensitive for the diagnosis of early AMI,but not superior to cTnT.Conclusion The present study showed that the plasma miR-133 a may be useful biomarker for AMI but is not superior to cTnT for the diagnosis of AMI.
