考试应激对医学生心身反应唾液免疫球蛋白及皮质醇的影响

【摘要】 目的探讨考试应激对医学生心身反应、唾液免疫球蛋白(SIgA)及皮质醇(Cor)的影响,为高校开展心理健康教育工作提供参考。方法整群抽取山西医科大学三年级临床专业医学生307名,运用考试焦虑量表进行测试,并采集考试期间与平时学习期间有效唾液样本进行生理指标的测试。结果医学生考试期间轻度焦虑者占43.3%,中度焦虑者占43.0%,较高水平焦虑者占13.7%;有躯体不适感者占51.5%。考试期间与正常学习期间相比,医学生SIgA水平有所降低,Cor水平有所升高(P值均<0.05)。考试焦虑与身体不适感呈正相关(P<0.05)。SIgA-c、Cor-c及身体不适感有随考试焦虑程度的加重呈升高的趋势。女生考试焦虑、Cor-c及身体不适感得分均高于男生。结论考试应激对大学生的心理、生理方面均有重要影响。在高校的心理卫生工作中,应以心理干预为切入点,同时辅以生理方面的健康教育或治疗。

【Abstract】 Objective To explore effects of exam stress to psychosomatic response and saliva immunoglobulin A(SIgA)and saliva cortisol(Cor) among medical college students.Methods Psychological questionnaires including Test Anxiety Scale(TAS) and Physical Discomfort Questionnaire were implemented.Physiological indexes including SIgA and Cor were collected and tested among 307 medical college students in Shanxi.Results During the examination period,about 43.3%,43.0% and 13.7%students reported mild,moderate and severe anxiety,respectively.More than half of students showed physical distress.SIgA level decreased and Cor level increased significantly after examination period.Test-related anxiety positively related to physical discomfort.No correlations were found in test-related anxiety with SIgA and Cor.IgA-c,Cor-c and physical discomfort increased with test-related anxiety.Girls had higher test-related anxiety,Cor-c and physical discomfort compared with boys.Conclusion Exam stress has important effects on psychological and physiological health among college students.Psychological intervention as well as health education should be emphasized in mental health promotion in universities.
