10734例女性泌尿生殖道支原体感染及耐药性分析
Analysis on the infection and drug resistance of urogenital tract mycoplasma in 10734 female cases
【摘要】 目的了解本地区女性泌尿生殖道解脲脲原体(Uu)和人型支原体(Mh)的感染及耐药状况。方法收集2012年7月至2013年6月到郑州大学第三附属医院就诊并疑似感染的女性患者样本10 734例,分析Uu及Mh的感染及耐药状况。结果 10 734例送检样本中,支原体阳性率55.52%,单独感染Uu最多(85.44%),其次为混合感染(13.32%),单独Mh感染最少(1.24%)。女性支原体总阳性率及单独Uu感染率在19~50岁年龄段较高;混合感染率在41~50岁年龄段较高;单独Mh感染率在各年龄段间无统计学差异。药敏显示单独Uu感染时,耐药情况较混合感染及单独Mh感染轻。四环素类药物敏感率大于90%;红霉素类抗生素在单独Uu感染时较敏感,单独Mh感染时除交沙霉素外均高度耐药。喹诺酮类药物对Uu及Mh有不同程度耐药,需进行药敏筛选。结论本地区女性单独Uu感染及Uu合并Mh的混合感染在不同年龄段分布不同。四环素类抗生素可作为本地区治疗女性泌尿生殖道支原体感染的首选用药,为控制耐药性应同时结合药敏结果用药。
【Abstract】 Objective The aim of this study was to investigate the infection and drug resistance of Ureaplasma urealyticum(Uu) and Mycoplasma hominis(Mh) among female population in Zhengzhou. Methods Ten thousand seven hundred and thirty-four samples were used to analyze the infection and drug resistance of Uu and Mh. These samples were taken from the woman who visited the third affiliated hospital of zhengzhou university and took mycoplasma tests from July 2012 to June 2013. Results The mycoplasma infection rate among female was 55.52%. Among the positive cases, Uu was the main pathogen and accounted for 85.44%, while mixed infection accounted for 13.32%, and Mh infection accounted for 1.24%. The total infection rate and single Uu infection rate among female between 19 and 50 years old were higher than those among other age groups. The mixed infection rate among female between 41 and 50 years old was higher than that among other age groups. Among different age groups, the single Mh infection rate was not different. Compared with mixed infection and Mh infection, drug resistance of Uu was less serious. Antibiotic sensitive rate of mycoplasma toward tetracyclines were over 90%. Excepted for josamycin, erythromycins were effective against Uu but not Mh. Uu and Mh were resistant towards quinolones in varying degrees, and antibiotic screening test should be done in advance. Conclusion For different ages, the mycoplasma infection rate among female was different. Regarding the treatment, tetracyclines should be used as the first choice of drug in this area. At the same time, antibiotic screening test also should be done for drug resistance control.
