微量MIC检测结核分枝杆菌药物敏感性的应用评价研究
Evaluation of the application of trace MIC on the examination of drug sensitivity of Mycobacterium tuberculosis
【摘要】 目的评价微量液体培养基最低抑菌浓度(MIC)快速法检测临床分离株耐多药结核分枝杆菌(Mycobacterium Tuberculosis,MTB)的临床应用价值。方法采用微量液体培养基MIC法对220株MTB临床分离株的链霉素(SM)、异烟肼(INH)、利福平(RFP)、乙胺丁醇(EMB)、氧氟沙星(OFLX)、左氧氟沙星(LVFX)、莫西沙星(MOX)、阿米卡星(AMK)、卡那霉素(KAN)和卷曲霉素(CPM)的最小抑菌浓度在96孔U型板微量液体培养基MIC中药物敏感性进行检测,并同传统的罗氏比例法药物敏感性进行比对。结果微量液体培养基MIC法药物敏感7~10 d获得结果,与罗氏比例法药敏结果进行比较,其敏感性分别为93.0%、95.9%、100.0%、93.8%、90.8%、88.6%、100.0%、90.0%、85.7%和93.7%,特异性分别为96.3%、95.9%、97.9%、94.9%、96.5%、97.7%、99.1%、93.9%、99.0%和95.2%,符合率分别为95.0%、95.9%、98.6%、94.5%、94.5%、95.9%、99.1%、93.2%、98.2%和95.0%。结论微量MIC药物敏性快速检测方法与罗氏比例法药物敏感性结果比较,10种药物微量MIC测定敏感性、特异性、符合率均较高,是目前较理想的一种快速检测结核分枝杆菌药物敏感性的方法,具有较高的临床应用价值。
【Abstract】 Objective The objective of the study was to assess the clinical value of trace liquid medium minimum inhibitory concentration(MIC) on the examination of isolated multidrug-resistant Mycobacterium tuberculosis. Methods Trace liquid medium MIC was used to determine the MIC of 220 isolated MTB strains, including streptomycin(SM), isoniazid(INH), rifampicin(RFP), ethambutol(EMB), ofloxacin(OFLX), levofloxacin(LVFX), moxifloxacin(MOX), amikacin(AMK), kanamycin(KAN), and capreomycin(CPM). Drug sensitivity of the strains was examined in 96-well round-bottom plates by trace liquid medium MIC, and was compared with results obtained by conventional Roche method. Results Comparison of the results obtained by trace liquid medium MIC on days 7-10 with those obtained by the Roche method showed that the sensitivities were 93.0%, 95.9%, 100%, 93.8%, 90.8%, 88.6%, 100%, 90.0%, 85.7%, and 93.7%, respectively; the specificities were 96.3%, 95.9%, 97.9%, 94.9%, 96.5%, 97.7%, 99.1%, 93.9%, 99.0%, and 95.2%, respectively; and the consistencies were 95.0%, 95.9%, 98.6%, 94.5%, 94.5%, 95.9%, 99.1%, 93.2%, 98.2%, and 95.0%, respectively. Conclusion Sensitivity, specificity, and consistency of the trace MIC method in evaluating the 10 antibiotics are higher than those of the Roche method, suggesting that trace MIC is an ideal method for the rapid detection of Mycobacterium tuberculosis and is high in clinical value.
