【摘要】 目的:评估120 W绿激光经尿道前列腺"倒八字法"切除术治疗良性前列腺增生(BPH)的安全性及疗效。方法:通过随访实施经尿道绿激光"倒八字法"切除术的63例患者,对平均手术时间、术后膀胱冲洗时间、术后留置导尿管时间、手术并发症、创面愈合时间等指标进行分析,对国际前列腺症状评分(IPSS)、生活质量评分(QOL)、剩余尿量及尿流率改变等指标进行手术前后对比。结果:平均手术时间(45.7±5.9)min,术后膀胱冲洗时间(10.0±1.2)h,术后留置导尿管时间(2.0±0.5)d,创面均于3个月内完全愈合,无严重并发症发生。IPSS及QOL评分术前平均为(26.0±1.9)及(6.0±0.6)分,于术后3个月分别下降至(9.6±1.6)及(2.7±0.5)分,最大尿流率术前为(6.9±1.7)ml/s,于术后3个月增加至(16.8±4.4)ml/s,剩余尿量术前为(163.7±19.9)ml,于术后3个月下降至(20.1±9.6)ml,手术前后比较差异均有统计学意义(P<0.05)。结论:经尿道前列腺绿激光"倒八字法"汽化切除术与选择性绿激光汽化术(PVP)相比具有效率高、可留取标本、手术时间短等优点,是治疗BPH(尤其对于大体积前列腺)的一种安全有效的微创手术。更多还原

【Abstract】 Objective:To evaluate the clinical efficacy and safety of 120 W green laser transurethral resection of the prostate with the " reverse Chinese character’8’" technique in the treatment of benign prostatic hyperplasia(BPH).Methods:We treated 63 patients with obstructive benign prostatic hyperplasia using a 120 Wgreen laser with the "reverse Chinese character’8’" technique.Mean operative time,bladder irrigation time,catheter time,surgical complications,wound healing time were measured and the following variables were assessed before and after operation:IPSS,QOL,residual urine volume and maximum urinary flow rate.Results:Mean operative time was(45.7±5.9)min,bladder irrigation time was(10.0±1.2)h,catheter time was(2.0±0.5)d,wound healing time was all within 3months.No severe complications occurred during perioperative period.Before and after operation,IPSS and QOL decreased from(26.0±1.9)and(6.0±0.6)to(9.6±1.6)and(2.7±0.5),maximum urinary flow rate increase from(6.9±1.7)mL/s to(16.8±4.4)mL/s,residual urine volume decreased from(163.7±19.9)mL to(20.1±9.6)mL.All results showed significant differences(P<0.05).Conclusions:Green laser transurethral resection of the prostate with the " reverse Chinese character’8’" technique was inferior to photoselective vaporization(PVP),has the advantages of high efficiency,obtaining specimens,short operation time,and was a safe and effective minimally invasive treatment for BPH(especially for large volume prostate).更多还原
