肝内信号肽酶复合物表达情况与慢性乙型肝炎临床特征的相关性

【摘要】 目的本研究探索信号肽酶复合物(SPC)在HBeAg(+)慢性乙型肝炎及HBeAg(-)慢性乙肝炎(CHB)患者肝脏中的表达是否存在区别,并结合两类患者肝组织病理改变(炎症、纤维化)、HBV DNA水平、血清HBeAg水平、肝功能损害水平等指标,期望为研究SPC与CHB发展、转归之间的关系提供一些实验室依据。方法选HBeAg(+)患者30例为A组,HBeAg(-)患者30例为B组。采集患者的性别、年龄数据;检查两组患者的肝功能(ALT、AST、TBIL)、血清HBV-DNA定量、血清HBeAg定量;对两组患者行肝穿刺病理活检,确定其炎症程度、纤维化程度;并对肝组织切片行SPC免疫组化染色,根据Shimizu标准作SPC染色积分统计,以了解SPC在肝内的表达。进行统计学分析对比两组患者上述数据的差异。结果 A组肝组织中SPC表达程度、炎症程度、HBV-DNA水平、血清e抗原滴度、ALT水平、AST水平均较B组高,差异有统计学意义(P<0.05)。而纤维化程度、年龄则较B组低,差异有统计学意义(P<0.05)。两组在性别和TBIL升高幅度上差异无统计学意义(P>0.05)。使用Spearman分析,发现SPC积分与HBeAg滴度呈正相关(r=0.704,P<0.01);与HBV-DNA呈正相关(r=0.571,P<0.01);与ALT呈正相关(r=0.587,P<0.01);与AST呈正相关(r=0.511,P<0.01);与TBIL呈负相关(r=-0.289,P=0.25);与炎症程度呈正相关(r=0.421,P<0.01);与纤维化程度呈负相关(r=0.655,P<0.01)。结论 SPC在肝组织中的表达定量与血清HBeAg滴度呈正相关,并与ALT/AST升高幅度、肝组织病理改变、血清病毒载量、e抗原滴度等相关。

【Abstract】 Objective To observe the difference of SPC expression in liver cells between HBeAg negative chronic hepatitis B(CHB) patients and HBeAg positive CHB patients,and to compare the data of SPC expression degree,liver pathological change,serum HBV-DNA quantitation,serum HBeAg quantitation,and the liver function in CHB patients in two types of CHB patients in order to provide some reference materials for study of the relationship between SPC expression and prognosis of CHB. Methods 30 cases of HBeAg positive CHB patients were chosen as group A,and 30cases of HBeAg negative CHB patients were as group B. The data of the patients’ sex,age were gathered. Their liver function(including ALT,AST,TBIL),HBV DNA quantitation,serum HBeAg quantitation were tested. Liver puncture was done to make clear the degree of inflammation and the degree of fibrosis. Immunohistochemical staining was used to check the SPC expression,and the SPC integral according to Shimizu standard was calculated. Then the above data of two groups were compared. Results SPC expressive degree,degree of inflammation,serum HBV DNA quantitation,serum HBeAg quantitation,ALT level,and AST level in group A were higher than those in group B. The differences were statistically significant(P < 0. 05). The differences of sex and TBIL level between two groups were statistically insignificant(P > 0. 05). Degree of fibrosis and age in group A was lower than those in group B. The differences were statistically significant(P < 0. 05). The relationship between SPC expression and serum HBeAg quantitation was positive correlation(r = 0. 704,P < 0. 01); the relationship between SPC expression and HBV-DNA quantitation was positive correlation(r = 0. 571,P < 0. 01); the relationship between SPC expression and ALT was positive correlation(r =0. 587,P < 0. 01); the relationship between SPC expression and AST was positive correlation(r = 0. 511,P < 0. 01);the relationship between SPC expression and TBIL was positive correlation(r =-0. 2894,P = 0. 25); the relationship between SPC expression and degree of inflammation was positive correlation(r = 0. 421,P < 0. 01); the relationship between SPC expression and degree of fibrosis was positive correlation(r = 0. 655,P < 0. 01). Conclusion The relationship between SPC expression in liver tissue and serum HBeAg quantitation is positive correlation. And a close relationship exists in SPC and age,ALT/AST level,pathological change,serum HBV-DNA quantitation,and serum HBeAg quantitation.
