长脉冲1064nm Nd:YAG激光与二氧化碳激光及液氮冷冻治疗化脓性肉芽肿的疗效比较

【摘要】 目的评价长脉冲1064 nm Nd:YAG激光、二氧化碳激光及液氮冷冻治疗化脓性肉芽肿的疗效及安全性。方法将137例化脓性肉芽肿患者随机分为3组,分别采用长脉冲1064 nm Nd:YAG激光(A组)、二氧化碳激光(B组)及液氮冷冻(C组)治疗,评估治疗后6个月的疗效及安全性。结果长脉冲1064 nm Nd:YAG激光、二氧化碳激光及液氮冷冻治疗化脓性肉芽肿的1次治愈率分别为86.9%、78.3%和57.8%,总治愈率分别为100%、86.9%和97.7%,不良反应发生率分别为4.3%、15.2%和13.3%。3种方法治疗化脓性肉芽肿安全有效,其中1次治愈率及总治愈率以Nd:YAG激光组最好,不良反应差异无统计学意义。结论长脉冲1064 nm Nd:YAG激光治疗化脓性肉芽肿安全有效。

【Abstract】 Objective To compare the clinical efficacy and safety of Nd:YAG laser with CO 2 laser and liquid nitrogen cryosurgery in the treatment of granuloma pyogenicum. Methods One hundred and thirty-seven cases with granuloma pyogenicum were divided into three groups, and were treated with long-pulsed Nd:YAG laser(Group A, 46 cases), CO 2 laser(Group B, 46 cases) and liquid nitrogen cryosurgery(Group C, 45 cases), respectively. Efficacy and safety evaluations were carried out 6 months after treatment. Results The cure rates after one treatment session were 86.9%, 78.3% and 57.8% in group A, B and C, respectively. The total cure rates in group A, B and C were 100%, 86.9% and 97.7%, respectively. The incidence of adverse events was 4.3%, 15.2% and 13.3% in group A, B and C respectively. All the three treatments were safe and effective in the treatment of granuloma pyogenicum, however, Nd:YAG laser treatment was the best when concerning about the total cure rate the cure rates after one treatment. There was no statistically significant difference adverse reactions. Conclusion Nd:YAG laser treatment was an effective and safe method for the treatment of granuloma pyogenicum, which could be use as a first-line choice.
