软组织肉瘤的CT特征与组织分化程度的对照研究

【摘要】 目的 探讨软组织肉瘤(STS)的CT表现与组织分化程度的影像病理学关系。方法 收集经病理证实的42例STS,对其大小、形态、密度、肿瘤与周围组织的关系进行回顾性CT影像分析,探讨这些征象在评价肿瘤组织分化程度上的价值。结果 36例瘤体最大横径>5 cm;29例呈卵圆形,13例呈不规则形;32例密度不均匀,其中瘤内钙化3例,瘤内坏死22例;25例侵犯周围组织。与病理学结果对照,瘤内坏死及肿瘤侵犯周围组织与肿瘤的组织学分化程度之间存在相关性(P<0.05),相关系数分别为0.64和0.57(P<0.01)。结论 瘤内坏死、肿瘤侵犯周围组织与STS的组织学分化程度相关,可在一定程度上反映低分化或未分化STS的生物学特征。

【Abstract】 Objective To investigate the relationship between CT characteristics and histological differentiation of soft tissue sarcomas.Methods Forty-two cases with pathologically proved soft tissue sarcomas were collected.The tumor size,morphology,density and relationship of the tumor to the adjacent structures on CT were analyzed retrospectively.The value of the CT signs in evaluating the histological differentiation of the tumor was explored.Results The maximal diameter of the tumor was over 5 cm in 36 cases.Oval shape was seen in 29 cases and irregular shape was seen in 13 cases.Heterogeneous density was seen in 32 cases including intratumoral calcification in 3 cases and intratumoral necrosis in 22 cases.Adjacent structures were infiltrated in 25 cases.Compared to the pathological results,intratumoral necrosis and invasion of adjacent structure’s were related to the degree of histological differentiation(P<0.05),and the relation coefficients were 0.64 and 0.57,respectively(P<0.01).Conclusion Intratumoral necrosis and invasion of adjacent structures are correlated with the histological differentiation of the soft tissue sarcomas.They may reflect the biological behavior of low-differentiated or undifferentiated soft tissue sarcomas to some extent.
