早期胃癌中原代细胞ssDNA适配子检测的临床意义
Clinical Significance of Detection of Primary Cell ssDNA Aptamer in Early Gastric Adenocarcinoma
【摘要】 目的探讨早期胃癌中原代细胞ssDNA适配子检测的临床意义。方法对5个标志物应用适配子-生物素-链霉亲和素磁珠分离系统进行分离、纯化及鉴定,筛选出特异性、灵敏度高的肿瘤标志物,并对其建立相应的检测方法,采用美国临床实验室标准化组织的相关标准对其检测方法进行临床评价,并参考《临床检验方法学评价》对其诊断性能进行分析。结果早期胃癌中原代细胞ssDNA适配子的检测敏感性为49.13%,特异性为91.44%,阳性似然比为5.74,阴性似然比为0.56 g,干扰实验51.8%,验前比65.7%,验后比197.2%。与金标准相比,在敏感性、特异性、准确度和精确度方面均无明显差异(P>0.05)。结论早期胃癌中原代细胞ssDNA适配子的检测,具有较高的灵敏度和较强的特异性,为早期胃癌的发现提供依据,可考虑作为早期胃癌的辅助诊断标准之一。
【Abstract】 Objective To analyze the clinical significance of detection of primary cell of ssDNA aptamer in early gastric adenocarcinoma. Methods The tumor markers of specificity and high sensitivity were selected by the method of separation,purification and identification based on the 5 markers using the established aptamer-mildew avidin biotin-chain magnetic bead,and set up a corresponding test method,used the clinical laboratory standardization organization’s standards on its detection methods for clinical evaluation,and referred to the clinical test methodology to evaluate and analyze the diagnostic performance. Results Sensitivity of the primitive cells ssDNA aptamer in early gastric cancer was 49. 13%,specificity was 91. 44%,positive likelihood ratio was 5. 74,negative likelihood ratio was 0. 56 g,interference experiment was 51. 8%,pre-test odds was 65. 7%,and post-test odds was 197. 2%. Compared with the gold standard,there were no obvious difference in sensitivity,specificity,accuracy and precision( P >0.05). Conclusion The detection of primitive cells ssDNA aptamer in early gastric cancer has high sensitivity and strong specificity,it provides the basis for detection of early gastric cancer,and can be considered as one of the auxiliary diagnostic criteria of early gastric cancer.
