外周血CD64 index对细菌感染性疾病的诊断价值

【摘要】 目的 评价CD64 index在诊断细菌感染性疾病中的应用价值。方法 61例感染发热病人,以白细胞分类和病原体分离培养结果为依据分为细菌感染组32例和病毒感染组29例,随机选择同期的健康体检者29例作为正常对照组;采用流式细胞术检测外周血中性粒细胞、淋巴细胞CD64的表达荧光强度水平来计算CD64 index,同时检测血清CRP含量;CD64 index与CRP诊断价值的比较采用ROC曲线来分析,采用四格表计算其敏感度、特异度、阳性似然比、阴性似然比,采用spearman相关性分析来分析CD64 index与CRP的相关性。结果 细菌感染组CD64 index为4.33±2.24,明显高于病毒感染组(2.83±1.36)和正常对照组(1.96±0.24)(P<0.05)。ROC曲线确定CD64 index诊断细菌性感染的最佳阈值为2.215;CD64 index的AUC为0.953,明显高于CRP(AUC=0.794,P<0.05)。CD64 index诊断细菌性感染的灵敏度、特异度和准确性分别为87.5%,86.2%和86.9%,高于CRP。而二者联合检测可提高其诊断的敏感度(97.3%)。CD64 index与CRP在细菌感染组和病毒感染组的相关性都很低,相关系数分别为0.186和0.181(P>0.05)。结论 外周血CD64 index对细菌性感染有较高的诊断价值,其诊断效能明显优于CRP。

【Abstract】 Objective To explore the diagnostic value of CD64 index for bacterial infections.Methods 61 patients with fever infection were divided into bacterial group(n=32) and virus group(n = 29) according to white blood cells and cultured classification;29 healthy controls were recruited as control group.CD64 was detected by flow cytometry(FCM).Serum level of CRP was detected at the same time.All results were compared and the ROC curve was used to analysis the value of CD64 index in diagnosis of bacterial infections.Results The CD64 index in bacterial group was 4.33±2.23,which was significantly higher than virus group(2.83±1.36) and control group(1.96±0.24)(P<0.05).The optimal threshold of CD64 index was 2.215 according to ROC curve;and the AUC of CD64 index was 0.953,which markedly increased compare to CRP(P<0.05).The sensitivity,specificity and accuracy of CD64 index were 87.5%,86.2%and 86.9%respectively,which significantly higher than CRP(P<0.05).CD64 index had no correlation with the levels of CRP in bacterial group(r=0.186,P>0.05) and virus group(r = 0.181,P>0.05).Conclusion CD64 index has a good diagnostic value for bacterial infection,which the diagnostic performance was significantly better than CRP.
