【摘要】 目的通过对产妇实施导乐分娩、腰硬联合无痛分娩、导乐配合无痛分娩干预产程过程,观察三种不同方式对产程时间、顺产率、剖宫产率的影响,探讨临床工作中如何通过以上三种方法,提高顺产率、降低剖宫产率。方法选择2007年1月-2012年12月入住泰安市中心医院产科的940例产妇,将其分为导乐分娩组、腰硬联合无痛分娩组、导乐配合无痛分娩组,以及未给予干预的对照组,将各组的总产程时间、顺产率、剖宫产率分别与对照组进行比较,观察其总产程时间、顺产率、剖宫产率的不同。结果导乐分娩组顺产率182(91%),剖宫产率18(9%),产程时间(7.8±1.20)h;对照组顺产率120(60%),剖宫产率80(40%),产程时间(13.8±1.27)h;腰硬联合无痛分娩组顺产率138(92%),剖宫产率12(8%),产程时间(6.12±1.25)h,对照组顺产率96(64%),剖宫产率54(36%),产程时间(13.1±2.08)h;导乐配合无痛分娩组顺产率113(94%),剖宫产率12(8%),产程时间(6.00±1.46)h,对照组顺产率73(61%),剖宫产率47(39%),产程时间(12.1±2.00)h。结论通过实施导乐分娩、腰硬联合无痛分娩、导乐配合无痛分娩,能够提高顺产率,降低剖宫产率,缩短产程时间。更多还原

【Abstract】 Objective To observe the influence of three different interventions,i. e. Doula delivery,pain-free delivery with combined spinal and epidural analgesia,Doula with pain-free delivery,on the duration of labor,the rate of eutocia and cesarean section in delivery,so as to explore the way to use the three methods to increase eutocia rate and to lower cesarean section rate. Methods A total of 940 puerperas who were admitted to the department of obstetrics in our hospital from Jan. 2007 to Dec. 2012 were selected and randomly divided into group of doula delivery,group of pain-free delivery with combined spinal and epidural analgesia,group of Doula with pain-free delivery,and control group. All the former three groups were compared with control group for the duration of labour,eutocia rate,cesarean section rate. Results The eutocia rate,the cesarean section rate,and the duration of labor were 91%(182),9%(18) and(7. 8 ± 1. 20) h respectively in group of Doul delivery; and they were 60%(120),40%(80) and(13. 8 ± 1. 27)h respectively in control group. The eutocia rate,the cesarean section rate,and the duration of labor were92%(138),8%(12) and( 6. 12 ± 1. 25)h respectively in group of pain-free delivery with combined spinal and epidural analgesia; and they were 64%(96),36%(54) and(13. 1 ±2. 08)h respectively in control group. The eutocia rate,the cesarean section rate,and the duration of labor were94%(113),8%(12) and(6. 00 ± 1. 46) h respectively in group of Doula with pain-free delivery; and they were 61%(73),39%(47) and(12. 1 ± 2. 00)h respectively in control group. Conclusions It can increase the rate of eutocia,lower the rate of cesarean section and shorten the duration of labor by applying Doula delivery,pain-free delivery with combined spinal and epidural analgesia,and Doula with pain-free delivery.更多还原
