阴茎坏死后缺损的个体化修复

【摘要】 目的 探讨阴茎坏死后缺损的个体化修复方法及疗效。方法 自2004年1月开始,对收治的18例阴茎坏死的患者,根据其阴茎坏死后残端的长度,采用不同的方法进行修复。结果 所有患者均获随访0.5~10.0年,其中6例阴茎残端小于3.0 cm者,术后皮瓣全部成活,再造阴茎的外形及感觉功能恢复良好,供区瘢痕不明显,外观形态良好;10例阴茎残端大于3.0 cm者,术后阴茎常态下延长(5.8±1.2)cm,勃起后延长(6.2±2.2)cm,周径增加(6.3±1.9)cm。随访早期,发生皮瓣远端坏死1例,切口裂开1例,经相应处理后均获痊愈,无感觉及勃起功能障碍发生;2例阴茎海绵体及尿道海绵体部分坏死缺损的患者,术后1个月排尿正常,随访未见异常。结论 针对阴茎缺损程度来制定规范化的诊疗方案,并进行个体化的修复,是手术成功的关键。

【Abstract】 Objective To investigate the standardized repair method and effect for the penile defect caused by necrosis.Methods Eighteen cases repaired by different methods according to the residual length after penile necrosis from January 2004 to now.Results Follow-up was performed for 0.5 to 10.0 years on 18 patients.For 6 cases with less than 3.0 cm penile residual length,all the flaps survived completely,and the reconstructed penis in all cases recovered with natural contour,good functions and concealed scar at the donor site;for 10 cases with more than 3.0 cm penile residual length,the length and the circumferences of the penis gained after operation were(5.8 ± 1.2)cm,(6.2± 2.2)cm and(6.3 ± 1.9)cm,respectively.During the early follow-up,there were no feeling and erectile dysfunction except for 2 case with necrosis of flap distant and disruption of the wound,respectively,which were recovered by reconstruction treatment;the other 2 cases were with corpora cavernosa and corpus spongiosum partial defect,normal urination could carry out at 1 month and there was no other abnormalities.Conclusion According to the characteristics of the penile defect,standardized and individual repair method is the key to successful treatment.
