彩色多普勒血流显像对肾动脉起始段狭窄支架术后的疗效评价

【摘要】 目的:应用彩色多普勒血流显像(CDFI)评价肾动脉起始段狭窄支架术后的疗效。方法:对128例肾动脉起始段狭窄的患者行经皮血管内球囊成形支架置入术,分别于术前、术后6个月行CDFI检查,观察术前及术后6个月肾动脉起始段、叶间动脉的收缩期峰值流速(PSA)、舒张期流速(EDV)、血流阻力指数(RI)、肾动脉起始段与腹主动脉峰值流速比值(RAR)及起始段管径变化。统计术后6个月狭窄再发生率。评价肾动脉起始段狭窄支架术后的疗效。结果:肾动脉起始段PSV和EDV由术前的(382.20±194.50)cm/s、(120.30±66.3 0)cm/s降为(98.40±39.40)cm/s、(29.01±13.60)cm/s(P<0.01)。叶间动脉PSV和EDV由术前的(18.20±7.40)cm/s、(8.01±2.40)cm/s升为(24.80±7.50)cm/s、(9.01±2.80)cm/s(P<0.01);RI由(0.56±0.10)升为(0.64±0.10)(P<0.01);RAR由(3.81±0.68)降为(1.26±0.22)(P<0.01);起始段管腔内径由(1.42±0.55)mm增为(3.60±0.52)mm(P<0.01)。6个月后起始段狭窄再发生12例,发生率9.4%。结论:CDFI可以有效地评价肾动脉支架治疗术后疗效,及时发现再狭窄的发生。

【Abstract】 Objective:To evaluate the therapeutic effects of stent implantation in stenosis of the initial segment of the renal artery by using color doppler flow imaging(CDFI).Methods:A totle of 128patients with stenosis of the initial segment of the renal artery underwent percutaneous transluminal angioplasty.CDFI was performed in these patients before and 6months after the operation.The data was collected and ananlyzed,including the peak systolic velocity(PSV),end-diastolic velocity(EDV),blood flow resistance index(RI)、renal aortic ratio(RAR)and changes in diameter of the initial segment of the renal artery and interlobar artery.The rate of stenosis recurrence was obtained 6months after operation.Results:Six months after operation,PSV and EDV of the initial segment of renal artery were decreased from(382.20±194.50)cm/s and(120.30±66.30)cm/s before the operation to(98.40±39.40)cm/s,(29.01±13.60)cm/s(P<0.01).PSV and EDV of interlobal artery were increased from(18.20±7.40)cm/sand(8.01±2.40)cm/s before the operation to(24.80±7.50)cm/s and(9.01±2.80)cm/s(P<0.01).RI of interlobal artery was increased from(0.56±0.10)to(0.64±0.10)(P<0.01).RARwere decreased from(3.81±0.68)to(1.26±0.22)(P<0.01);the diameter of the initial segment of renal artery has increased from(1.42±0.50)mm to。(3.60±0.52)mm(P<0.01).Six months after the operation,stenosis re-occurred in12 out of 128 cases and the rate of recurrence was 9.4%.Conclusion:CDFI can be used to evaluate the therapeutic effects of stent implantation of the renal artery and detect the recurrence of stenosis in time.
