七氟醚麻醉饱和状态后低流量的可行性(英文)
The feasibility of low fresh gas flow following saturation of sevoflurane anesthesia
【摘要】 目的在七氟醚达到饱和状态并维持一段时间后降低氧流量,以期获得稳定的麻醉深度,并减少七氟醚的用量。方法 20例腹部肿瘤患者(ASA I级),拟在七氟醚吸入全麻联合硬膜外麻醉下手术。潮气量(tidal volum,TV)设置为8 mL/kg,I:E=1:1.5,RR=10 BPM,新鲜气体流量(fresh gas flow,FGF)=分钟通气量(minute ventilation,MV),设定浓度(delivered fraction,Fd)为2.4%vol。呼气末浓度(end-tidal fraction,Fet)达到峰值10 min后,记录吸入浓度(inspired fraction,Fi)的数值。将FGF/MV从1降低到0.4,10 min内每隔1 min记录Fi和Fet的数值;然后将FGF/MV从0.4降低到0.3,同样方法记录Fi和Wet;最后将FGF/MV从0.3降低到0.2,同样方法记录Fi和Fet的数值。结果 Fet达到峰值后,降低FGF/MV引起FI轻度下降;相对于起始状态(0 min),在2和10 min时略有下降,但差异均无统计学意义,呼气末吸入浓度可以稳定在1.7%vol。结论在七氟醚吸入麻醉达到饱和状态后可以使用低流量维持。
【Abstract】 Objective To explored whether it was feasible to use low fresh gas flow(FGF) during sevoflurane anesthesia when a state of saturation was achieved,and to reduce the dosage of sevoflurane.Methods Twenty patients with abdominal neoplasms of ASA I,scheduled to undergo surgical operations under inhalation anesthesia of sevoflurane combined with epidural block.Tital volum(TV) was set at8 mL/kg,I:E= l:1.5,RR=10 BPM,FGF= minute ventilation(MV),and delivered fraction(Fd) = 2.4%vol.When end-tidal fraction(Fet) reached its maximum for more than 10 min.the inspired fraction(Fi) was recorded.FGF/MV was then decreased from 1 to 0.4,and FI and Fet were recorded every minute,which lasted for 10 min.FGF/MV was again decreased from 0.4 to 0.3,and Fi and Fet were recorded in the same manner.Finally,FGF/MV was decreased from 0.3 to 0.2,and Fi and Fet were similarly treated.Results After Fet had reached its maximum,a fall in FGF/MV caused a slight decrease in Fi and Fet at the time points of 2-min to 10-min compared with the initial state(0-min).However,no difference was observed among these time points,while Fet remained stable at 1.7%vol.Conclusions It is feasible to reduce FGF when the saturation state of sevoflurane anesthesia is achieved.
