【摘要】 目的:探讨腹腔镜下双侧卵巢子宫内膜异位囊肿剥除术中使用垂体后叶素注射对卵巢储备功能的影响。方法:对46例双侧卵巢子宫内膜异位囊肿患者随机分成研究组和对照组。研究组采用稀释的垂体后叶素注入到卵巢皮质与卵巢囊肿壁间隙后再行剥除,对照组采用常规剥除法。术前和术后超声检测卵巢窦卵泡数(AFC)和卵巢体积。结果:研究组术中出血量和手术时间均少于对照组,差异有统计学意义(P<0.01)。两组术后AFC值和卵巢体积均较术前明显下降,与术前比较差异有统计学意义(P<0.01)。研究组术后AFC减少率[(14.34±1.17)%]少于对照组[(38.98±16.42)%],差异有统计学意义(P<0.01)。研究组术后卵巢体积缩小率[(27.84±6.86)%]少于对照组[(53.46±9.92)%],差异有统计学意义(P<0.01)。结论:腹腔镜下双侧卵巢子宫内膜异位囊肿剥除术会导致卵巢储备功能的降低,在术中增加使用垂体后叶素注射对卵巢储备功能的影响相对减少。更多还原

【Abstract】 Objective: To investigate the effect of vasopressin injection technique used in laparoscopic cystectomy of bilateral ovarian endometrioma on ovarian reserve. Methods: 46 patients with bilateral ovarian endometrioma were allocated randomly into study group and control group. Diluted vasopressin was injected into the space between the normal ovarian cortex and the cyst wall before stripping in the study group,and ordinary laparoscopic cystectomy without injection was adopted in control group. Antral follicle count( AFC) and ovarian volume were measured by ultrasound before and after surgery. Results: Bood loss and operative time of study group were significantly less than those of control group( P<0. 01). There were statistically significantly greater decrease in postoperative AFC and ovarian volume as compared with the preoperative values in both groups( P<0. 01). AFC reduction rate of study group[( 14. 34±1. 17) %]was less than control group[( 38. 98±16. 42) %],and the difference was statistically significant( P<0. 01).Ovarian volume reduction rate of study group[( 27. 84±6. 86) %]was less than control group[( 53. 46 ±9. 92) % ],and the difference was statistically significant( P<0. 01). Conclusions: Ovarian reserve function was decreased during stripping bilateral ovarian endometrioma under laparoscopy.
