脑室内常见肿瘤的磁共振鉴别诊断
MRI Differential Diagnosis of Intraventricular Common Tumors
【摘要】 目的分析总结脑室内常见肿瘤的磁共振影像学特点及其鉴别诊断,以提高诊断准确率。方法回顾性分析34例经病理证实的脑室内肿瘤患者的临床表现及磁共振征象。结果 34例患者中男性儿童和青少年患者所占比例较大,发生于侧脑室最多,透明隔病变最少。脉络丛乳头状瘤仅见于侧脑室内、室管膜瘤第四脑室多见、生殖细胞瘤蝶鞍内多见、颅咽管瘤第三脑室多见、脑膜瘤和星形细胞瘤发生部位多变。MRI以等/长T1、等/长T2信号为主,短T1、短T2信号出现率较低,且仅见于颅咽管瘤和脑膜瘤。所有病例增强扫描多数强化不均匀,4例无强化;钙化囊变多见,出血少见;几乎都有脑积水、半数发生脑水肿。结论脑室内肿瘤有其特征性临床症状、磁共振信号改变及间接影像学征象。
【Abstract】 Objective To analysis and summary of magnetic resonance imaging features and differential diagnosis of the intraventricular common tumors in order to improve the diagnostic accuracy.Methods Make a retrospective analysis of the clinical manifestations and magnetic resonance signal of 34 cases of intraventricular tumor patients had been confirmed by pathology examination.Results A larger proportion of the children and adolescents in 34 cases of patients.The most of tumors located in the lateral ventricle and a few of the tumors in the septum pellucidum.Choroid Plexus Papilloma was found only in the lateral ventricle.Most of ependymoma in the fourth cerebral ventricle.Most of Germ Cell Tumor located in the sella turcica.Most of the craniopharyngeal duct tumor located in the third ventricle.Astrocytoma and Meningioma distribution did not feature.Most patients whose lesions was showed moderate/long Tl signals or moderate/long T2 signal in the MRI scanning,very few of patients are short Tl signal or short T2 signal.The signal of short Tl and short T2 appears only in craniopharyngioma and meningioma.Most of the lesions were enhancement inhomogeneous.There were 4 cases without signal enhancement.All lesions were often calcified and cystic degeneration,but the bleeding was relatively rare.All of the lesions had hydrocephalus mostly.Half of the lesions had cerebral edema.Conclusion There are characteristic clinical symptoms and directly or indirectly magnetic resonance signals in the intraventricular tumors.
