超声联合弹性成像对乳腺良恶性病灶的鉴别诊断
Ultrasound combined with elastography in the differentiation diagnosis of benign breast nidus from malignant one
【摘要】 目的探讨常规超声、超声弹性成像硬度评分法及面积比法对乳腺良恶性病灶的诊断价值,为超声鉴别乳腺良恶性病灶提供帮助。方法收集64例女性患者的67个乳腺病灶,采用飞利浦公司IU-22彩超诊断仪,探头频率5~12 MHz,配有弹性成像软件,分别行常规超声及超声弹性成像检查,判断病灶弹性硬度评分,计算病灶常规超声及超声弹性成像图的面积比。应用SPSS13.0软件包进行统计学分析,检查方法与病理结果之间比较用Kappa一致性检验,多种检查方法间两两比较用基于χ2分割法的多重比较。检验水准α=0.0045,以P≤0.05为差异有统计学意义。结果常规超声、弹性成像硬度评分法及面积比法、弹性成像硬度评分结合面积比法、常规超声联合弹性成像技术5种方法两两比较,联合诊断对乳腺恶性病灶诊断的准确性高于常规超声,评分法与面积比法比较差异有统计学意义(P≤0.05),UE硬度评分法+面积比法与常规超声+UE法间差异无统计学意义(P>0.05)。结论 1超声弹性成像硬度评分法和面积比法有助于乳腺病灶良恶性的鉴别,两者结合使诊断的准确性大大提高;2常规超声及弹性成像技术对乳腺病灶良恶性鉴别诊断有一定价值,但均存在一定的局限性,若两者联合应用,准确性明显提高,在临床应用中可以相互补充。
【Abstract】 Objective To investigate the value of conventional ultrasound,ultrasound elastography scores and area ratio method in the diagnosis of benign and malignant breast nidus. Methods The 67 lesions from 64 female subjects who were diagnosed as breast masses were collected in this study. The patients were examined with conventional ultrasound and ultrasound elasticity imaging technology. The nidus were evaluated by elastic scores,and the area ratio of the lesion was calculated. Five methods included conventional ultrasound,UE scores,area ratio method,UE scores combined with area ratio method,conventional ultrasound combined with elastography from which the obtained results were compared. Results The accuracy of combined diagnosis to mammary gland malignant pathological changes diagnosis was higher than that of conventional supersonic examination,the difference between UE scores method and area ratio method was statistically significant(P<0.05). The difference between UE scores combined with area ratio method and conventional ultrasound combined with elastography was not statistically significant(P>0.05). Conclusion The method of UE scores or the ratios of traced areas is helpful to differentiate the benign from malignant mammary gland nidus,the accuracy of combined diagnosis can be improved remarkably. Conventional ultrasound and ultrasound elastography technique for diagnosis of benign and malignant breast nidus are available,their combination can improve the diagnostic accuracy.
