【摘要】 目的 探讨静脉滴注藻酸双酯钠导致缺血性阴茎异常勃起的临床特点及其机制。方法 分析北大医院男科中心2010年至2012年收治的9例静脉滴注藻酸双酯钠后出现缺血性阴茎异常勃起患者的病史、临床表现特点、实验室检查及治疗。同时检索万方医学数据库文献,分析静脉滴注藻酸双酯钠导致的缺血性阴茎异常勃起的临床特点、治疗方法。结果 万方医学数据库1998年至2013年文献共报道49例静脉滴注藻酸双脂钠后发生的阴茎异常勃起病例。我院共收治9例静脉滴注藻酸双酯钠引起的缺血性阴茎异常勃起的患者,应用藻酸双酯钠24h内发病的3例,其他为应用藻酸双酯钠2~7d;9例患者血常规及凝血功能检查均无明显异常。文献中49例患者,应用藻酸双酯钠24h内发病3例,其他应用藻酸双酯钠2~20d,平均4~5d;报道血常规及凝血结果的6例,均无异常;2例报道阴茎海绵体内血液呈高凝状态。结论 静脉滴注藻酸双酯钠后,可能使阴茎海绵体局部血液粘滞状态改变,出现高凝状态,导致缺血性阴茎异常勃起发生。阴茎异常勃起出现后,应及时处理,尽早使用海绵体内缩血管药物注射、海绵体抽血或阴茎头阴茎海绵体分流术等方式处理。更多还原

【Abstract】 Objective To investigate the clinical characteristic and mechanism of ischemic priapism induced by intravenous Polysaccharide sulfate.Methods The medical history,characteristic of clinical manifestation,results of laboratory examination and treatments of nine patients with ischemic priapism induced by intravenous polysaccharide sulfate were comparatively analyzed and summarized.The case-reports of ischemic priapism in published literature of Wangfang Med database were reviewed.Results There were 49 case reports for patients with ischemic priapism induced by intravenous polysaccharide sulfate in Wanfang Med database.For nine patients treated in our hospital,3 patients had priapism during 24 hours after intravenous polysaccharide sulfate,the others had priapism during 2-7 days after intravenous polysaceharide sulfate,and their blood routine examination and coagulation function test results were normal.For the 49 patients from reviews,3 patients had priapism during 24 hours after intravenous polysaccharide sulfate,the others had priapism during 2-20 days after intravenous polysaccharide sulfate and the 2 patients had hypercoagulability.Conclusion After intravenous polysaccharide sulfate,the blood state in corpus cavernosum transitted to hypercoagulability,which resulted in priapism.The treatment to priapism should be proceeded in time.更多还原
