Ⅱ,Ⅲ期非小细胞肺癌术后肿瘤微血管密度检测及临床意义

【摘要】 目的研究Ⅱ,Ⅲ期非小细胞肺癌肿瘤组织微血管密度(MVD)对预后及术后治疗的影响。方法采用免疫组化S-P法,检测50例Ⅱ,Ⅲ期非小细胞肺癌组织、20例肺良性病变组织中的MVD。结果①肺癌组织中MVD(60.60±24.22)显著高于肺良性病变组织(11.85±2.72)(P<0.01);②MVD与非小细胞肺癌PTNM分期有密切关系(P<0.05),而与肺癌患者性别、年龄、吸烟史、肺癌原发部位、组织学类型、分化程度等均无明显关系(P>0.05);③肺癌组织中MVD高密度组(MVD≥50)患者5年生存率明显低于低密度组(MVD<50)5年生存率(P<0.01);④多因素C0X模型分析发现:PTNM分期和MVD是判断肺癌预后有价值的指标,而MVD更加精确。结论 MVD在肺癌的发生、发展、转移中起重要作用,可作为肺癌生物学行为的一项评估指标,检测肺癌中MVD有助于预测肺癌预后及指导术后治疗。

【Abstract】 Objective To research the effect of Ⅱ,Ⅲ stage non-small cell lung cancer tumor microvascular density(micro-vascular density,MVD) guidelines on the prognosis and postoperative treatment.Methods Fifty cases of Ⅱ,Ⅲ stage non-small cell lung cancer and 20 cases of pulmonary benign lesions MVD in the organization were detected through imrnunohistochemical S-P method.Results ①MVD in which lung cancer tissues(60.60 ± 24.22) was significantly higher than that of pulmonary benign lesions(11.85 ± 2.72);②The MVD had close relationship with PTNM staging non-small cell lung cancer(P <0.05),and lung cancer patients’ gender,age,smoking history,lung cancer primary site,histological type and differentiation degree had no obvious relation(P > 0.05);③In lung cancer tissue,the 5-year survival rate of MVD group(MVD≥50) was significantly lower than that of the.low density(MVD<50);④Multi-factor COX model analysis found that:PTNM staging and MVD were valuable indicators to judge the prognosis of lung cancer,and MVD was more accurate.Conclusion MVD in the occurrence,development and metastasis of lung cancer plays an important role,and it can be used as an evaluation index of the lung cancer biology behavior.The detection of lung cancer MVD can predict prognosis of lung cancer and guide the postoperative therapy.
