钠钾镁钙葡萄糖注射液(乐加)用于术中输液治疗的临床观察

【摘要】 目的 观察钠钾镁钙葡萄糖注射液(乐加)对胃肠道手术患者术中血生化和血流动力学的影响。方法 选择择期在全麻下行开放性胃肠道手术患者44例,随机分为试验组(乐加)和对照组(乳酸林格氏液)。麻醉诱导前输注500 ml实验液,随后以实验液8 ml/(kg·h)和万汶4ml/(kg·h)输注至手术结束。分别记录输液前即刻、诱导前即刻、诱导后1 h、2 h、手术结束时的血钾、镁、钙、乳酸、血糖及平均动脉压(MAP)、全心舒张末期容积指数(GEDI)、胸腔血容积指数(ITBI)。结果 输液后各时间点两组血钾、镁、钙、乳酸及MAP、GEDI、ITBI无统计学差异(P>0.05),两组血糖缓慢升高,输液后2 h及手术结束时试验组血糖水平明显高于对照组(P<0.05)。结论 乐加在维持血电解质、乳酸及血流动力学稳定方面与乳酸林格氏液相当,持续输注乐加引起的血糖升高较乳酸林格氏液明显。

【Abstract】 Objective To observe the effects of sodium potassium magnesium calcium and glucose injection on blood biochemistry tests and hemodynamics parameters during gastrointestinal operation.Methods Forty-four patients aged from 18 to 65 with ASA grade Ⅰ~Ⅱ scheduled for elective gastrointestinal surgery were randomly assigned to experimental group(sodium potassium magnesium calcium glucose injection)and control group(lactate Ringer’s injection).Each patient received 500ml infusion before the induction,and then infused at the rate of 8ml/(kg·h)until the surgery was finished.Voluven was infused at the rate of 4ml/(kg·h)until the surgery was finished.Blood electrolytes,lactate,glucose,mean arterial pressure(MAP),Global End-diastolic Volume Index(GEDI) and Intrathoracic Blood Volume Index(ITBI)were measured before infusion,before induction,1h after induction,2h after induction and at the end of surgery.Results There were no significant intergroup differences in blood electrolytes,lactate,MAP,GEDI or ITBI at each time point.Blood glucose was increased when the injections were infused in both groups,and the level of blood glucose in experimental group was significantly higher than that in control group at 2h after induction and the end of surgery.Conclusion Sodium potassium magnesium calcium and glucose injection has similar effects on maintenance of blood electrolytes,lactate,MAP,GEDI and ITBI as lactate Ringer’s,but causes more obvious hyperglycemia.
