原发性肾病综合征水肿患者肾组织水通道蛋白的表达
The expression of aquaporins in renal tissue of primary nephrotic syndrome patients with edema
【摘要】 目的 探讨肾组织水通道蛋白(AQP)的表达与原发性肾病综合征(NS)水肿的关系。方法 将54例经肾活检病理检查证实为原发性肾小球疾病的患者分为3组,即非NS组(A组)、NS不伴水肿组(B组)和NS伴水肿组(C组),应用免疫组化法检测3组患者肾组织AQP1、2、3、4的表达。结果 AQP1主要表达于近端小管,C组患者AQP1阳性面积百分比为(0.414±0.201)%,显著低于A组的(0.683±0.311)%及B组的(0.652±0.300)%(P均<0.05),A、B组间表达比较差异无统计学意义(P>0.05);AQP2主要表达于集合管,C组AQP2阳性面积百分比为(0.823±0.002)%,显著高于A组的(0.512±0.213)%(P<0.01)及B组的(0.665±0.228)%(P<0.05),A、B两组间表达比较差异有统计学意义(P<0.05);AQP3亦主要表达于集合管,C组AQP3阳性面积百分比为(0.614±0.114)%,较A组的(0.585±0.111)%及B组的(0.592±0.119)%有升高趋势,但差异无统计学意义(P>0.05);AQP4仅在部分患者肾组织集合管有表达。结论 水通道蛋白在肾组织中的表达异常与原发性肾病综合征患者的水肿有关,其中AQP1、AQP2可能参与了水肿的发生,AQP3、AQP4的作用尚待进一步探讨。
【Abstract】 Objective To explore the relation between the expressions of aquaporins in renal tissue and edema in patients with primary nephrotic syndrome(NS).Methods Fifty-four primary NS patients,diagnosed by renal biopsy,were divided into three groups:non-NS group(Group A),NS without edema group(Group B) and NS with edema group(Group C).Expressions of AQP1,AQP2,AQP3 and AQP4 in renal tissues of all patients were detected by immunohistochemistry.Results AQP1 mainly expressed in proximal tubule.The proportion of AQP1 positive area in group[(0.414 ±0.201)%was significantly lower than that in group A(0.683 ±0.311)%or group B(0.652 ±0.300)%,but there was no significant difference between group A and group B.AQP2 and AQP3 mainly expressed in collecting duct.The proportion of AQP2 positive area in group C(0.823 ±0.002)%was significantly higher than that in group A(0.512 ±0.213)%or group B(0.665 ±0.228)%.meanwhile the difference between group A and group B was also significant.The proportion of AQP3 positive area in group C(0.614 ±0.114)%was higher than that in group A(0.585 ±0.111)%or group B(0.592 ±0.119)%,but the difference was not statistically significant.AQP4 expressed in collecting duct only in part of the patients.Conclusion The expression of aquaporins in renal is related to the edema in PNS patients.AQP1 and AQP2 might play an important role in the edema of PNS patients,while the association between AQP3,AQP4 and edema in PNS patients needs to be investigated further.
