非小细胞肺癌患者癌组织和血浆中microRNA-21的表达及意义
Expression and significance of microRNA-21 in the cancer tissue and plasma of patients with non-small cell lung cancer
【摘要】 目的探讨microRNA-21(miRNA-21,miR-21)在非小细胞肺癌(NSCLC)患者肺癌组织和血浆中的表达及临床意义。方法用荧光定量PCR检测100例NSCLC患者肺癌组织及癌旁组织miR-21的相对表达量,并检测100例NSCLC患者及100例健康人血浆miR-21的相对表达量,用t检验比较其表达水平的差异,用卡方检验分析肺癌组织miR-21表达量与各临床病理资料之间的关系,ROC曲线评价miR-21对NSCLC的诊断价值。结果 miR-21在肺癌组织的表达水平高于癌旁组织(t=10.154,P<0.05),且在NSCLC患者血浆中的表达水平高于健康人(t=13.444,P<0.05);肺癌组织中miR-21高表达患者肺癌复发转移率明显高于低表达患者(χ2=11.335,P<0.05);生存分析显示,肺癌组织或血浆中miR-21高表达患者较低表达者生存期明显缩短(χ2为6.685和5.692,P<0.05);ROC曲线表明,miR-21对NSCLC诊断价值中等。结论 miR-21在NSCLC患者癌组织及血浆中均有高表达,肺癌组织miR-21高表达与NSCLC复发转移具有密切关系,可作为NSCLC诊断及评价预后的指标。
【Abstract】 Objective To investigate the expression level of microRNA-21( miR-21) in the cancer tissue and plasma of patients with non-small cell lung cancer( NSCLC) and evaluate its clinical significance. Methods The relative expression levels of miR-21 in lung cancer tissues and tumor-adjacent normal tissues of 100 patients with NSCLC and in plasma of 100 patients with NSCLC and 100 healthy controls were detected by real-time PCR. The difference in the expression levels of miR-21 between groups was compared with t test,and the correlations between the expression level of miR-21 in lung cancer tissues and various clinical and pathological data were analyzed with Chi square test. The diagnostic value of miR-21 in NSCLC was evaluated with ROC curve. Results The expression level of miR-21 in lung cancer tissues was significantly higher than that in tumor-adjacent normal tissues( t = 10. 154,P < 0. 05) and the expression level of miR-21 in plasma of patients with NSCLC was obviously higher than that in healthy controls( t = 13. 444,P <0. 05). The recurrence and metastasis rates in patients with high levels of miR-21 in lung cancer tissues were significantly higher than that in patients with low levels of miR-21( χ2= 11. 335,P < 0. 05). Kaplan-Meier analysis showed that the patients with high levels of miR-21 in lung cancer tissue or plasma had a short survival time( χ2= 6. 685,P < 0. 05; χ2= 5. 692,P < 0. 05). The ROC curve indicated that miR-21 had an acceptable value in the diagnosis of NSCLC. Conclusion MiR-21 is highly expressed in lung cancer tissue and plasma of patients with NSCLC,and the high expression of miR-21 in lung cancer tissue is closely related to the recurrence and metastasis of NSCLC,indicating that miR-21 may be used as a diagnostic and prognostic indicator of NSCLC.
