【摘要】 目的探讨肺肉芽肿性多发血管炎的MSCT表现特征及其演变过程。方法回顾性分析经临床确诊的7例肺肉芽肿性多血管炎患者的临床及MSCT资料(其中4例有完整的随访记录),总结7例患者(39个病灶)的影像特征及其变化规律。结果 39个病灶中,12个呈结节状改变,9个呈肿块状改变,14个呈空洞样改变,4个呈实变样改变;结节、肿块或空洞边缘均呈"毛刺"样改变,病灶周围呈"晕环"状改变者18个,病灶近心侧见"营养血管影"者31个,结节、肿块或实变病灶内增强扫描见"血管强化征"者15个。结论肺内多发性结节、肿块、空洞或实变病灶,若病灶边缘呈"毛刺"样改变,周围见"晕环"状改变,近心侧见"供养血管征"或病灶内见"强化血管影",则高度提示肺肉芽肿性多发血管炎的可能性。更多还原

【Abstract】 Objective To study the MSCT characteristics of pulmonary granulomatous polyangiitis and to analyze their dynamic changes. Methods The clinical data and MSCT findings of 7 patients with clinically-confirmed pulmonary granulomatous polyangiitis were retrospectively analyzed. Among them,4 patients had complete follow-up record. A total of 39 lesions were detected in the 7 cases,the imaging features and their dynamic changes were evaluated. Results The 39 lesions were characterized by nodule( n = 12),mass( n = 9),cavitation( n = 14) and consolidation( n = 4). Marginal"burr"and"halo ring"sign around the lesion were observed in 18 lesions,and"nutrient vessel"shadow at lesion’s radial side was seen in 31 lesions. On enhanced CT scanning,"enhancement vessel"sign within the nodule,mass or consolidation was demonstrated in 15 lesions. Conclusion Multiple pulmonary nodules,masses,cavitation or consolidations,marginal "burr","halo ring"sign around the lesion,"nutrient vessel"shadow at lesion’s radial side,"enhancement vessel"sign within the lesion,etc.,all these signs are strongly suggestive of pulmonary granulomatous polyangiitis.更多还原
