256层CT对IDD型矫正型大动脉转位的解剖及临床分析

【摘要】 目的利用256层iCT对先天性心脏病IDD型矫正型大动脉转位(CCTGA)进行影像分析,归纳总结其螺旋CT成像中表现影像学特点及其临床意义。方法搜集整理并回顾分析我院8例确诊为IDD型CCTGA患者的临床及螺旋CT影像资料,患者均行256层螺旋CT、超声心动图及心脏平片检查,对照分析并归纳总结患者的影像及临床资料。结果 256层iCT对于本组病例全部做出了正确诊断且清晰地显示了多种合并的复杂畸形,其中7例合并室间隔缺损(VSD),1例合并房间隔缺损(ASD),4例合并明显的肺动脉瓣或流出道狭窄(PS),4例合并主动脉窦转位,3例合并双上腔静脉畸形,1例合并动脉导管未闭(PDA),2例患者超声心动图发现较为明显的三尖瓣关闭不全。结论多层螺旋CT能够清晰地显示IDD型矫正型大动脉转位的解剖学关系及其合并的相关畸形,对其诊断具有极高的正确率。

【Abstract】 Objective To explored clinical application of the 256 slice spiral CT in Corrected transpostion of great arteries with artrial siti inversus,and summarized the imaging characteristic performance. Methods The clinical and imaging data of 8 patients with congenitally corrected transpostion of great arteries( CCTGA) were collected and retrospective analyzed. All the patients underwent CT,echocardiography and heart plain film examination. Results All patients had made the right diagnosis by 256 slice spiral CT. The accompanied multi malformation also been diagnosis and displayed clearly,included ventricular septal defect in 7 cases,patent ductus arteriosus in 1 case,atrial septal defect in 1 case,obvious pulmonary or pulmonary outflow tract stenosis in 4 cases,coronary sinus translocation in 4 cases,double superior vena cava malformation in 3 cases. Two patients were found obvious tricuspid insufficiency by echocardiography. Conclusion Multi slice CT can display the anatomic abnormalities and accompanying deformity in corrected transpostion of great arteries with artrial siti inversus with high diagnostic accuracy.
