下颌角弧形截骨线的解剖定位与临床应用

【摘要】 目的:为下颌角弧形截骨线提供实用的定位方法。方法:100例(200侧)成人离体下颌骨的下颌管暴露。测量下颌管关键点距下颌缘的距离;观测截骨线关键点与下颌孔、下颌牙的对应关系;依据观测结果设计出符合个性化需求的弧形截骨线;在X线片上进行验证。结果:下颌管关键点下颌孔、磨牙后三角和下颌角间连线与下颌管的交点、经磨牙后三角向下颌体所引垂线与下颌管的交点、位于颏孔与经磨牙后三角向下颌体所引垂线之间下颌管的中点、颏孔距下颌缘的距离分别为(14.02±2.23)mm、(21.06±3.90)mm、(14.55±3.34)mm、(13.60±2.8)mm、(14.08±3.68)mm,各点的最大截骨宽度分别为(10.05±4.00)mm、(15.06±2.88)mm、(8.08±3.88)mm、(6.09±3.45)mm、(7.06±3.56)mm;截骨线的后端点与下颌孔、下牙槽弓位于同一平面的几率为96%;3条截骨线的前端点分别与下颌支前缘、下颌第2磨牙、下颌第2前磨牙相对应。结论:该弧形截骨线的定位方法,符合解剖学特征和美学要求,具有标志明确,术后效果好的优点。

【Abstract】 Objective:To provide positioning methods for the curved osteotomy line on the mandibular angle.Methods:100cases(200 sides) of mandible tube in the mandible bone of adults were exposed;The distance from the critical dot of the mandibular canal to marginal mandibular were measured;The correspondence between the key point on the osteotomy with the mandibular foramen and the mandibular teeth was observed;The curved osteotomy line was designed based on observations to meet the individual needs,which was verified on X-ray.Results:The distance between the key points of mandibular canal,mandibular foramen,intersection point of the line from retromolar triangle to mandibular angle and mandibular canal,intersection point of the vertical line from retromolar triangle to mandibular margin and mandibular canal,and the midpoint of mandibular canal from mental foramen to the vertical line from retromolar triangle to mandibular margin,mental foramen and the mandibular edge was(14.02±2.23) mm,(21.06±3.90) mm,(14.55±3.34) mm,(13.60±2.8) mm,(14.08±3.68) mm,respectively;the maximum width each was(10.05±4.00) mm,(15.06±2.88) mm,(8.08±3.88) mm,(6.09±3.45) mm,(7.06±3.56) mm;the probability that the endpoint A of the osteotomy line and mandibular foramen and inferior alveolar of the bow was located in the same plane falls on 96%;the front point of three osteotomy line,fall respectively on the point intersection where the vertical line led downward via the edge of the ramus,and the mandibular second molar,the mandibular second premolar.Conclusion:The positioning method of the curved osteotomy line on the mandibular angle may comply with anatomical features and aesthetic requirements which takes the advantages as easyoperation with clear signs and preferable effect after operation.
