年龄和性别差异对心外膜脂肪体积与冠心病相关性的影响

【摘要】 目的国内外越来越多的研究证明心外膜脂肪组织(epicardial adipose tissue,EAT)与冠心病的发生及进展密切相关。本研究通过运用双源CT(dual-source CT,DSCT)心脏成像测定心外膜脂肪体积(epicardial adipose tissue volume,EATV),探讨EATV和冠心病及其危险因素的关系以及其在不同年龄和性别人群中的差异。方法采集2010年4月至2012年6月301例接受DSCT心脏冠状动脉造影且接受介入冠状动脉造影检查的患者资料,①根据造影结果冠状动脉狭窄程度≥50%、<50%分为冠心病组和非冠心病组;②依据年龄≥75岁、60~74岁、<60岁分为高龄、中龄、低龄3组;③根据性别分组,比较同一性别组内冠心病和非冠心病患者各指标的差异。结果①冠心病患者EATV明显增加,EATV是影响冠心病发生的危险因素。②低、中、高龄组间EATV分别为(88.05±35.46)cm3、(103.85±33.53)cm3和(117.51±34.81)cm3,两两比较,差异有统计学意义(P<0.05);冠心病发生率均呈现出随年龄增加的递增趋势(从低到高分别为55.1%、75.9%和89.1%,P<0.05)。③低龄组男女EATV[(90.81±35.92)cm3vs(70.41±30.07)cm3]和冠心病发生率(60.7%vs 37.9%)差异存有明显统计学意义(P<0.05),其他各年龄组内两性间EATV和冠心病发生率均无统计学意义(P>0.05)。④男性中冠心病和非冠心病EATV值差异有统计学意义[(111.41±32.33)cm3vs(98.30±31.23)cm3,P=0.026]。结论 EATV是影响冠心病发生的危险因素,EATV随着年龄增加呈现递增趋势,在>60岁的患者中EATV不存在性别差异,EATV对男性冠心病发生的影响较女性更显著。

【Abstract】 Objective More and more studies have shown the close correlation of epicardial adipose tissue( EAT) with the occurrence and progression of coronary heart disease. This study aimed to investigate the association of EAT volume( EATV) with coronary heart disease and its risk factors in different age and gender groups. Methods We collected the data of 301 patients undergoing dualsource CT( DSCT) coronary angiography and interventional coronary angiography from April 2010 to June 2012,with EATV measured by DSCT and coronary heart disease( CHD) diagnosed by the criterion of coronary artery stenosis ≥50% on interventional coronary angiography. We divided the patients into a CHD and a non-CHD group based on coronary artery stenosis,into an old( ≥75years),a middleaged( 60- 75 years) and a young group( < 60 years) according to age,and into a male and a female group based on gender. We compared the clinical parameters between the CHD and non-CHD groups within the same gender group. Results EATV was evidently increased in the CHD patients. The EATV of junior,middle and senior were( 88. 05 ± 35. 46),( 103. 85 ± 33. 53) and( 117. 51 ± 34. 81) cm3 respectively,with significant differences among different age groups( P =0.001). The incidence rate of CHD rose with the increase of age from junior to senior groups were 55. 1%,75. 9% and 89. 1% respectively. While there were no statistically significant differences between males and females in EATV and the incidence of CHD in different age groups( P > 0. 05) except in the young subjects. Significant differences in EATV were observed between CHD and non-CHD groups in males( P = 0. 026),but not in females( P > 0. 05). Conclusion EATV is an risk factor of CHD,and it rises with the increase of age( [111. 41 ± 32. 33]cm3vs [98. 30 ± 31. 23]cm3),with no difference between men and women older than 60 years. EATV seems more closely associated with CHD in males than in females.
