【摘要】 目的探讨术中置入空肠营养管对于防治胃癌术后胃瘫患者的重要作用。方法对天津第五中心医院普外科2009年7月~2013年7月235例胃癌手术患者进行回顾性分析,统计术中置入空肠营养管165例(实验组)和未置入营养管70例(对照组),比较两组患者残胃功能指标:初始容积、初始压力、最大耐受容积、最大耐受压力、顺应性;比较两组患者胃瘫发生率及治愈时间。结果术后发生胃瘫实验组9.09%(15/165)<对照组18.57%(13/70);两组患者残胃最大耐受容积、最大耐受压力及顺应性有显著性差异(P<0.05)。平均胃瘫治愈时间实验组(12 d)<对照组(21 d)。结论术中置入空肠营养管对胃瘫高危患者不仅起到预防及治疗作用,还缩短胃瘫恢复时间及住院时间,降低住院费用,对胃癌患者的康复有积极现实意义。更多还原

【Abstract】 Objective To investigate the important functions of placement of jejunal nutrient canal during surgery on the prevention of gastroparesis after stomach cancer operation. Methods Retrospective analysis was conducted among 235 patients of stomach cancer operation in the department of general surgery of Tianjin Fifth Central Hospital,in which,165 cases of placement of jejunal nutrient canal during surgery( experimental group) and 70 cases of those without the placement( contrast group) was carried out. Comparisons of indicators of stomach functions of two groups was made,which were original volume,original pressure,maximum tolerance volume,maximum tolerance pressure,compliance and comparisons of incidence rates of gastroparesis and cure time. Results 9. 09%( 15 /165) of incidence of gastroparesis after surgery in the experimental group was smaller than 18. 57%( 13 /70) in the contrast group;There were significant differences between two groups of patients with residual stomach in the aspects of maximum tolerance volume,maximum tolerance pressure and compliance( P <0. 05). Average cure time of gastroparesis of experimental group was shorter than that of contrast group( 12 d vs 21 d). Conclusion Placement of jejunal nutrient canal during surgery for patients of gastroparesis in danger not only has preventive and therapeutic effects,but also shortens periods of recovery and hospital admission and reduces hospital expenses,which is very meaningful to cancer patients.更多还原
