高频彩色多普勒超声在高血压患者颈动脉粥样硬化诊断中的应用

【摘要】 目的探讨高频彩色多普勒超声在高血压患者颈动脉粥样硬化诊断中的应用。方法将123例诊断为高血压的患者作为观察组,并将同期在我院进行体检的60例健康成人作为对照组,两组人群均进行高频彩色多普勒超检查。结果观察组的颈动脉(CCA)内径(d),颈内动脉(ICA)d、搏动指数(PI)及阻力指数(RI)均显著高于对照组,观察组的舒张末期血液速度(EDV)及收缩期峰值流速(PSV)均显著低于对照组,P<0.05;观察组与对照组的动脉内-中膜厚度(IMT)的主干厚度分别为(0.12±0.05)cm和(0.09±0.03)cm,分叉厚度分别为(0.13±0.09)cm和(0.10±0.03)cm,斑块形成率分别为63.4%和18.3%,两两比较,均P<0.01,对照组的管腔狭窄情况显著优于观察组,P<0.01。结论高频彩色多普勒超声在诊断高血压患者颈动脉粥样硬化可有效识别颈动脉硬化位置、斑块情况及评价血管的狭窄程度。

【Abstract】 Objective To explore the application of High- frequency Color Doppler Ultrasound in the diagnosis of hypertension combined with carotid atherosclerosis. Methods 123 patients with hypertension are regarded as the observation group,and 60 healthy adults are regarded as the control group. All patients were carried out by High- frequency Color Doppler Ultrasound. Results The carotid artery( CCA),internal carotid artery( ICA) internal diameter( d),pulsatility index( PI) and resistance index( RI) were significantly higher than that in the control group. The end diastolic blood velocity( edv) and peak systolic velocity( psv) were significantly lower than that in the control group,P < 0. 05. The intima- media thickness( IMT) trunk thickness in the control group and the observation group was( 0.12 ±0.05 cm vs 0.09 ±0.03 cm),the bifurcation thickness was( 0.13 ±0.09 cm vs 0.10 ±0.03 cm),and plaque formation rate was( 63.4% vs 18.3%),P < 0.01. The stenosis in the control group was significantly better than the observation group,P <0. 01. Conclusion High-frequency Color Doppler Ultrasound in the diagnosis of hypertension combined with carotid atherosclerosis can effectively identify the location of carotid atherosclerosis and plaque,and evaluate stenosis of blood vessels.
