跟骨锁定钢板治疗跟骨关节内骨折(附33例报告)

【摘要】 目的评价锁定钢板内固定治疗跟骨关节内骨折的疗效。方法自2009年6月-2013年6月共收治33例33足跟骨闭合性关节内骨折,右足18例,左足15例。高处坠落伤27例,车祸伤6例。均为闭合性骨折。根据Sanders分型:Ⅱ型14足,Ⅲ型16足,Ⅳ型3足。均行跟骨外侧入路应用锁定钢板内固定手术治疗,术后不用石膏固定,早期功能练习。采用美国足踝外科协会(AOFAS)踝与后足功能评分评价足踝功能。结果随访时间6~15个月,平均9个月。骨折愈合时间10~14周,平均11.5周;2例出现切口裂开,经治疗后好转。末次随访时AOFAS踝与后足评分系统评价踝关节功能:优17足,良13足,可3足,优良率为90.9%(30/33)。结论锁定钢板治疗跟骨骨折有利于骨折早期愈合和功能练习,软组织并发症发生率较低。

【Abstract】 Objective To evaluate the clinical outcome of calcaneal locking plates for treatment of severely displaced intra-articular calcaneal fractures.Methods From June 2009 to June 2013,33 patients( 23 males and 10 females) with severely displaced intra-articular calcaneal fractures were treated by open reduction and internal fixation using locking plates.The left side was involved in 15 cases,and the right side in 18 cases.According to Sanders classification,14 cases belonged to type Ⅱ,16 cases to type Ⅲ,and 3 to type Ⅳ.The results were validated using the American Orthopaedic Foot and Ankle Society( AOFAS) ankle-hindfoot scale.Results Altogether the 33 patients got a mean follow-up of 9( 6 to 15) months.All fractures healed in a mean time of 11.5( 10 to 14) weeks.Incision dehiscence occurred in two cases,but healed after treatment.According to AOFAS Foot Score standard,the effects of 17 cases were excellent,13 were good,and 3 were fair.The excellent and good rate was 90.9 %( 30 / 33).Conclusion The application of locking plate is an effective treatment for calcaneal fractures,which is beneficial for healing and functional exercise,and the rate of soft tissue complication after operation is low.
