【摘要】 目的探讨腹腔镜下输尿管切开取石术治疗输尿管上段结石的临床疗效及其安全性。方法回顾性分析2007年1月-2011年1月在我院行输尿管切开取石术治疗的输尿管上段结石患者42例资料,根据手术方式分为2组:对照组接受传统的开放术式,观察组给予后腹腔镜手术治疗。术后随访6~12个月,比较2组患者的近期临床疗效。结果与对照组相比,观察组患者的手术时间、术中出血量、输血量显著降低(P<0.05)。治疗组术后平均住院时间、手术医疗费用、下床活动时间显著低于对照组(P<0.05)。与治疗前相比,2组患者治疗后的体质量和卡氏评分显著提高(P<0.05),但观察组术后评分显著高于对照组(P<0.05)。观察组的不良反应发生率为2.9%显著低于对照组的20.0%(P<0.05)。结论与传统的开放术式相比,腹腔镜下输尿管切开术取石治疗输尿管上段结石疗效确切,手术创伤小,术后恢复快,近期疗效显著,结石清除率高,不良反应少。更多还原

【Abstract】 Objective To investigate the effect of laparoscopic ureterolithotomy in treatment of upper ureteral calculi and its safety. Methods 42 patients who underwent laparoscopic ureterolithotomy from January 2007 to January 2011 in our hospital were analyzed retrospectively. The cases were divided into two groups. The control group received traditional open surgery while patients in the observation group underwent laparoscopic retroperitoneal ureterlithotomy and were followed up for 6 ~ 12months. The short-term clinical effect in the two groups was compared. Results Compared with the control group,the operation time,the intraoperative blood loss and the blood transfusion volume in the observation group decreased significantly( P < 0. 05). The average postoperative hospital stay was significantly shorter,the medical cost significantly lower,and the time of outof-bed activity significantly less in the observation group than in the control group( P < 0. 05). The body mass and Karnofsky score in the two groups improved significantly after treatment( P < 0. 05),but the effects in the observation group were significantly higher than those in the control group( P < 0. 05). Adverse reactions rate was 2. 9% in the observation group,which was significantly lower than 20% in the control group( P < 0. 05). Conclusion Compared with traditional open surgery,laparoscopic ureterolithotomy in treatment of upper ureteral calculi has such advantages as exact curative effect,little trauma,faster postoperative recovery,significant short-term effect,high stone clearance rate,and less adverse reactions.更多还原
