CT检查鉴别结节性甲状腺肿和甲状腺癌的价值分析

【摘要】 目的:比较分析结节性甲状腺肿和甲状腺癌的CT表现并探讨CT检查的应用价值。方法:选取35例甲状腺结节的患者为研究对象,其中结节性甲状腺肿患者16例,甲状腺癌患者19例,均接受64层螺旋CT扫描,比较结节性甲状腺肿和甲状腺癌的CT表现。结果:结节性甲状腺肿:平扫密度均匀11例,边缘清晰7例,病灶密度均匀9例,边缘清晰6例;增强扫描密度均匀13例,边缘清晰15例,病灶密度均匀10例,边缘清晰12例;甲状腺癌:平扫密度均匀7例,边缘清晰5例,病灶密度均匀6例,边缘清晰2例;增强扫描密度均匀8例,边缘清晰10例,病灶密度均匀4例,边缘清晰5例;结节性甲状腺肿与甲状腺癌在数目、结节边缘规则与否、有无颈部肿大淋巴结、增强后边缘较平扫清晰或模糊及增强结节实性密度均匀与否方面比较有显著性差异(P<0.05)。结论:结节性甲状腺肿与甲状腺癌有各自的特异性CT表现,CT检查能在一定程度上准确诊断结节性甲状腺肿与甲状腺癌,值得在临床上进一步推广。

【Abstract】 Objective: To investigate the CT findings of nodular goiter and thyroid cancer and to explore the application value of CT examination. Method: 35 cases of patients with thyroid nodules were chosen for the study,including 16 patients with nodular goiter,thyroid cancer,19 patients underwent 64-slice spiral CT scan,the nodular goiter and thyroid cancer CT findings were analyzed. Result: Nodular goiter: plain 11 cases uniform density,7 cases sharp edges,9 cases uniform density,6 cases sharp edges; enhanced scan 13 cases uniform density,15 cases sharp edges,10 cases uniform density,12 cases edge clear; thyroid cancer: plain 7 cases uniform density,5 cases sharp edges,6 cases uniform density,2 cases sharp edges; enhanced scan 8 cases uniform density,10 cases sharp edges,4 cases uniform density,5 cases sharp edges; there were significant difference in the number of nodules edge rules or not,with or without cervical lymph nodes, enhanced edge than unenhanced and enhanced clarity or blurred solid nodules with relatively uniform density of nodular goiter and thyroid cancer( P < 0.05). Conclusion: Nodular goiter and thyroid cancer have their own specific CT findings,to a certain extent CT examination,accurately diagnose nodular goiter and thyroid cancer,it is worth in clinical further promotion.
