【摘要】 目的探讨可视喉镜在困难气道患者气管插管中的应用价值。方法选取2011年1月—2012年8月就诊于重庆市急救医疗中心拟行气管插管的困难气道患者40例,随机分为研究组和对照组,每组20例。研究组采用可视喉镜引导下行气管插管,对照组采用直接喉镜行气管插管。对两组平均动脉压(MAP)和心率(HR)进行监测,并记录比较气管插管时间和并发症发生情况。结果与本组诱导前相比,两组气管插管开始时MAP、HR均显著降低(P<0.05),但两组比较差异无统计学意义(P>0.05);插管完成时,研究组MAP、HR基本恢复正常,与对照组比较显著降低(P<0.05),而对照组MAP、HR仍显著高于诱导前(P<0.05)。与对照组相比,研究组气管插管总时间、声门最佳暴露时间显著缩短,经口置入导管时间显著延长,并发症发生率显著降低,差异均有统计学意义(P<0.05)。结论采用可视喉镜引导下气管插管是一种安全、有效的插管方式,可提高困难气道患者气管插管的成功率。更多还原

【Abstract】 Objective To explore the clinical significance of video laryngoscopes in patients with difficult airway intubation. Methods A total of 40 patients with difficult airways,who would undergo endotracheal intubation,during January2011 and August 2012 were randomly divided into study group( n =20) and control group( n =20). The study group underwent endotracheal intubation under video laryngoscopes,while the control group underwent direct laryngoscopy and tracheal intubation. The values of mean arterial pressure( MAP) and heart rate( HR) were monitored,and intubation time and the incidence rates of complications in the two groups were also recorded. Results Compared with those before the induction in the same group,the values of MAP and HR at the time of intubation in the two groups significantly decreased( P < 0. 05),but there was no statistically significant difference between the two groups( P >0. 05); at the time of intubation finish,values of MAP and HR in study group returned to normal,and the values were significantly decreased compared with those in control group( P <0. 05),while the values in control group were significantly higher than those before induction( P < 0. 05). Compared with those in control group,the total intubation time and the best glottal opening time were significantly reduced,intubation time through the mouth was significantly prolonged and incidence rates of complications were significantly lower,and the differences were statistically significant( P < 0. 05). Conclusion Endotracheal intubation under video laryngoscopes is safe and effective and can improve intubation success rate in patients with difficult airway.更多还原
