青年女性急脑梗死患者临床特点及危险因素分析
A study on clinical features and risk factors for young female patients with cerebral infarction
【摘要】 目的分析青年女性急性脑梗死患者的临床特点,探讨其主要危险因素。方法回顾分析108例2008年1月—2013年6月在本院治疗青年女性急性脑梗死患者的临床资料。采用多因素Logistic回归分析筛选青年女性急性脑梗死的危险因素。结果 108例患者中,36~45岁年龄段人数最多,占62.96%;83例(76.85%)的病变部位在基底节区;13例患者出现了并发症,发生率为12.04%,其中肺部感染最多有6例(5.56%)。多因素Logistic回归分析显示,高血压(OR=2.213,95%CI:1.322~3.712,P<0.001)、短暂性脑缺血发作(OR=3.893,95%CI:2.319~6.535,P=0.003)、颅内外动脉狭窄或闭塞(OR=8.954,95%CI:1.783~45.128,P<0.001)是青年女性急性脑梗死发病的独立危险因素。结论高血压、短暂性脑缺血发作、颅内外动脉狭窄或闭塞是导致青年女性脑梗死发生的最主要病因,控制好上述危险因素,对脑梗死的预防和治疗有重要临床意义。
【Abstract】 Objective To analyse the clinical features of young female patients with cerebral infarction and to explore the risk factors. Methods We retrospectively reviewed the charts of 108 young female patients diagnosed with cerebral infarction and treated in Puning Overseas Chinese Hospital during the period from January 2008 to June 2013. Multivariate Logistic regression analysis was used in screening risk factors of young female cerebral infarction patients. Results In 108 patients,the 36 to 45 age group was the largest,accounting for 62. 96%; there are 83 cases( 76. 85%) of the lesions in the basal ganglia; 13 patients had a complication,the rate was 12. 04%,in which lung infections was up to 6 cases( 5. 56%). Logistic regression analysis showed that hypertension( OR = 2. 213,95% CI: 1. 322 ~ 3. 712,P < 0. 001) transient is chemic attack( OR = 3. 893,95% CI: 2. 319 ~ 6. 535,P = 0. 003),intracranial artery stenosis or occlusion( OR = 8. 954,95% CI: 1. 783 ~ 45. 128,P < 0. 001) were independent risk factors for cerebral infarction. Conclusion Hypertension,transient is chemic attack,intracranial and extracranial artery stenosis or occlusion is the main cause of cerebral infarction in young women,control of these risk factors,prevention and treatment of cerebral infarction has important clinical significance.
