【摘要】 目的:通过对比干燥综合征(SS)腮腺CT造影、唇腺活检结果与临床诊断标准对诊断SS的灵敏度、特异度、一致率,以明确CT造影对SS的诊断价值。方法:选择我院2009年5月至2014年3月以来门诊及住院的可疑SS 130例,均采用GE Light speed MSCT平扫及单侧CT造影。采用多平面重建(MPR)、最大密度投影(MIP)和容积再现(VR)技术重建。将以上结果分别与临床诊断(含唇腺活检)相关结果标准作对照研究和统计学分析。结果:CT造影与唇腺活检两种检查方法诊断SS结果比较,差异有统计学意义(P=0.00)。按临床诊断标准确诊为SS 80例中,CT造影诊断SS阳性70例,阴性10例,灵敏度87.5%,特异度96.0%,一致率为91%,Youden’s指数83.5%,唇腺活检诊断SS阳性57例,阴性23例,灵敏度71.3%,特异度86.0%,一致率为76.9%,Youden’s指数57.3%,两者比较,差异有统计学意义(P<0.05)。结论:CT造影诊断SS灵敏度及特异度均较唇腺活检高,且与临床诊断标准具有高度一致性,可作为SS的最重要诊断方法之一。更多还原

【Abstract】 Objective:The results of CT sialography of parotid,labial gland biopsy and clinical diagnostic criteria of Sjogren’s syndrome were contrasted in terms of sensitivity,specificity and consistent rate,in order to investigate the value of CT angiography in the diagnosis of Sjogren’s syndrome.Methods:The data of 130 cases with suspected Sjogren’s syndrome in our hospital were analyzed from May 2009 to June 2014.All of the cases underwent MSCT scan and unilateral CT angiography.Multi-planar reconstruction,maximum intensity projection,and volume rendering technology were applied after CT scan.All of the results were compared with clinical diagnostic criteria(include labial gland biopsy)and the statistical analysis were performed.Results:Results of the two methods in diagnosis of Sjogren’s syndrome by comparison,the difference has statistical significance,P <0.05.There were 70 positive cases and 10 negative cases to be dianosed with the method of CT angiography and 57 positive cases and 23 negative cases with the method of labial gland biopsy from the 80 cases confirmed by clinical diagnostic criteria.The sensitivity,specificity,accuracy rate and Youden’s index of CT angiography(87.5%,96.0% and 91%)were significantly higher than those of labial gland biopsy(71.3%,86.0%and 91%)in dignosing Sjogren’s syndrome.The four indexes of the two methods,CT sialography of parotid and the labial gland biopsy,had statistical significance to diagnose Sjogren’s syndrome(P =0.013).Conclusion:The sensitivity and specificity of CT angiography were higher than those of lip biopsy in the diagnosis of Sjogren’s syndrome,with a high degree of consistency of with clinical diagnostic criteria,so that,CT angiography can be used as one of the most important method to diagnose Sjogren’s syndrome.更多还原
