CD44和maspin在肺鳞状细胞癌中的表达及其与血管生成的关系
Expression of CD44 and maspin in lung squamous cell carcinoma and their relationship with angiogenesis
【摘要】 目的检测肺鳞状细胞癌中CD44蛋白和maspin蛋白的表达及相互之间的关系,并研究它们与肺鳞状细胞癌患者临床病理因素之间的关系。方法采用免疫组织化学Elivision法检测116例肺鳞状细胞癌和30例正常肺组织中CD44、maspin和CD34蛋白表达情况。结果在肺鳞状细胞癌和正常肺组织中CD44和maspin蛋白的阳性表达率分别为60.3%、39.7%和10.0%、100%,两组之间相比,差异有统计学意义;CD44和maspin蛋白的阳性表达率和微血管密度(microvessel density MVD)与肺鳞状细胞癌患者肿瘤组织的分化级别、淋巴结转移与否以及临床分期等相关(P<0.05);且CD44蛋白阳性表达率与MVD计数呈正相关关系,maspin蛋白阳性表达与CD44蛋白阳性表达和MVD计数均呈负相关关系(P<0.05)。Kaplan-Meier生存分析显示,CD44蛋白阳性表达组患者的生存时间明显高低于其阴性表达组患者(P<0.001);maspin蛋白阳性表达组患者的生存时间明显高于其阴性表达组患者(P<0.001);MVD≥21组患者生存时间明显低于MVD<21组的患者(P<0.001)。多因素分析表明CD44和maspin蛋白的表达是肺鳞状细胞癌患者的独立预后因素。结论 CD44和maspin的异常表达参与了肺鳞状细胞癌的发生、发展、浸润和转移,联合检测CD44和maspin蛋白的表达对肺鳞状细胞癌的进展和预后判断有重要意义。
【Abstract】 Objective To explore the expression of CD44and maspin proteins in the lung squamous cell carcinoma(LSCC)and their relationship with microvessel density(MVD).Methods The expression of CD44,maspin,and CD34proteins was examined by immunohistochemistry in 116specimens of LSCC and 30normal lung tissues.Results The positive expression of CD44and maspin in normal lung tissues was 10.0%and 100%,but 60.3%and 39.7%,respectively in LSCC tissues.There was significant differences between the two groups(P<0.05).The expression of CD44and maspin proteins was significantly related with grades of tumors,lymph node metastasis,and TNM stages,as was the score of MVD(P<0.05).There was a positive relationship between the expression of CD44protein and the score of MVD(P<0.001);and the expression of maspin protein was negatively associated with the expression of CD44protein and the score of MVD(P<0.001).Kaplan-Meier survival analysis:showed that the overall mean survival time of patients with positive CD44protein expression was shorter than that of those with negative expression(P<0.001);the survival time patients with positive maspin expression was significantly longer than that of those with negative expression(P<0001);the survival time of patients with≥21was significantly shorter than that of those with MVD<21group(P<0.001).Positive expressions of CD44and maspin were independent prognostic factors of LSCC(P<0.05).Conclusion Abnormal expression of CD44and maspin plays an important role in the initiation,progression,and metastasis of LSCC.The combined detection of CD44and maspin has an important role in predicting the progression and prognosis of LSCC.
