683例新生儿感染性肺炎病原菌分布及药敏结果分析

【摘要】 目的:对我院不同年龄新生儿感染性肺炎的病原菌及药敏试验结果进行分析,为临床合理使用抗菌药物提供依据。方法:回顾性分析2009年1月至2013年9月我院儿科住院治疗的683例新生儿肺炎患儿痰培养及药敏试验结果。结果:683例患儿痰培养阳性者188例,占27.5%。早期新生儿组G-菌感染多见,晚期新生儿组G+菌感染多见。最常见的细菌为大肠埃希菌、肺炎链球菌、葡萄球菌属及流感嗜血杆菌。两种细菌混合感染者共43例,以晚期新生儿组多见。常见病原菌对头孢他啶、阿米卡星、环丙沙星和氨曲南、亚胺培南较为敏感。结论:大肠埃希菌是四川成都双流地区新生儿肺炎最常见病原菌。头孢他啶在四川成都双流地区新生儿肺炎中敏感率高、安全性好,可考虑作为四川成都双流地区新生儿肺炎治疗时首选药物。

【Abstract】 Objective: To study the pathogens distribution and drug resistance in neonatal infectious pneumonia in our hospital,and to provide the basis for clinical rational use of antibiotics. Methods: The data of sputum bacterial culture and antimicrobial susceptibility test of 683 cases with neonatal pneumonia were collected and analyzed retrospectively. Results: The overall positive rate of bacterial culture was 27. 5%; gram-negative infection was dominant in early newborn group,while gram-positive infection in lately newborn group. The common detected bacteria included Escherichia coli,Streptococcus pneumoniae,Staphylococcus and Haemophilus influenzae respectively. The mixed infection was more common in the lately newborn group than that the in early group. Majority strains were sensitive to ceftazidime,amikacin,ciprofloxacin,aztreonam and imipenem. Conclusions: The major strain in local area of neonatal pneumonia was Escherichia coli. Ceftazidime was safe and sensitive to majority bacteria; it was the first line medicine for neonatal pneumonia in local area.
