甘肃省合作市藏族农牧民不同生理阶段尿碘与甲状腺激素水平比较
Comparison of urinary iodine and thyroid hormone levels of Tibetan farmers and herdsman at different stages in Hezuo of Gannan Province
【摘要】 目的评价甘肃省合作市藏族人群的碘营养状况,掌握其不同生理阶段的甲状腺激素水平,为藏族地区碘缺乏病防治提供依据。方法随机选取在当地居住两年以上的藏族常住人口为受试对象,根据其生理阶段设定孕妇、哺乳期、未孕育龄妇女组(各30名),同时设定20~50岁的成年男子(90名)和8~10岁儿童组(90名)作为研究对象,采集随意一次尿样检测尿碘含量;空腹采集静脉血检测血清甲状腺激素水平。结果农区孕妇的尿碘中位数未达到150μg/L的推荐标准,不同人群尿碘之间差异有统计学意义(χ2=38.3,P<0.01);牧区孕妇和哺乳期妇女的尿碘中位数未达到150μg/L的推荐标准,不同人群之间尿碘值差异有统计学意义(χ2=72.83,P<0.01);牧区儿童TT3水平低于农区儿童,差异有统计学意义(t=9.98,P<0.01),TT4水平高于农区儿童,差异有统计学意义(t=4.00,P<0.01);牧区育龄妇女TT3(t=8.82)、TT4(t=2.22)、FT3(t=4.62)水平均低于农区育龄妇女(t=5.00,均P<0.01);牧区成年男性TT3和FT3水平高于农区成年男性,差异有统计学意义(t=4.76,P<0.01);不同生理阶段人群中各项甲状腺激素水平偏高者多于偏低者。结论在当前的盐碘水平下合作市5组不同生理阶段人群尿碘中位数都达到了相应的推荐标准,人群总体碘营养状况得到了有效纠正,但重点人群尿碘低于适宜水平的比例明显偏高,学生和成年男性尿碘高于适宜水平的比例偏高,说明甘南藏族农、牧民甲状腺激素水平现状不容乐观。
【Abstract】 Objective To evaluate iodine nutrition status of Tibetan people in Hezuo City of Gansu Province, to know their thyroid hormone at different physiological stages, provide basis for IDD prevention in Tibetan area. Methods Tibetan residents inhabited over two years were selected randomly, and divided into groups according to their physiological status of pregnant, lactating, unpregnancy of childbearing age(30 persons each), as well as groups of adult male of 20-50 year-old(90persons) and children of 8-10 year-old(90 children) as the research object. Urine samples were collected randomly to detect for iodine content, and serum thyroid hormone was determined by collecting fasting venous blood. Results The median of urine iodine(MUI) of pregnant women in farming and pastoral areas and women at lactation in pastoral area didn’t meet the recommended standard of 150 μg/L. There was statistical difference in urine iodine in various crowds(χ2=38.3,χ2=72.83,P<0.01). TT3 of children was lower in pastoral area than in farming area, with statistical significance(t=9.98,P<0.01), while their TT4 was higher than that of children in farming area with statistical significance(t=4.00,P<0.01). TT3(t=8.82), TT4(t=2.22) and FT4( t=4.62) of women at reproductive age were lower in pastoral area than in farming area( t=5.00, all P<0.01).TT3 and FT3 of the male was higher in pastoral area than in farming area with statistical significance(t=4.76,P<0.01). There were more people at the different stages with high level of thyroid hormone. Conclusions Under current level of salt iodine, median of urinary iodine of five groups at different physiological stages in Hezuo City reaches the appropriate recommended standard. The overall iodine nutritional status is effectively corrected. But proportion of the urinary iodine of focus groups less than appropriate level is high, and also as the students and adult man. The investigated situation shows thyroid hormone levels in Gannan Tibetan pastoral and farming areas needs changing.
