【摘要】 目的:研究腰椎间盘突出症患者手法治疗前后,脊柱对称性指标与症状学评分间的相关性。方法:采集70例腰椎间盘突出症患者保守治疗前及治疗20d后症状学评分指标,包括:直腿抬高角度(SLR)、视觉疼痛量表(VAS)、日本骨科学会腰突症病情评分(JOA)等,同时使用投影栅轮廓测量技术检测脊柱对称性指标,采用SAS 9.0软件将两者进行相关性分析。结果:治疗前后脊柱对称性指标的变化值与症状评分改善值呈正相关关系。其中侧屈角比LR、侧弯角M的变化与症状学评分改善的相关系数较大。侧屈角比LR和旋转角HA权重最大。结论:治疗前后侧屈角之比LR、侧弯角M、旋转角HA,尤其是LR与患者临床症状变化具有显著相关性,可以客观阐明脊柱的功能状态和病情恢复程度;手法治疗腰椎间盘突出症应该重视脊柱力学平衡因素。更多还原

【Abstract】 Objective: To explore the relevance between the spinal symmetry index and some quantii ed indexes for the clinical manifestation of the lumbar disc herniation(LDH) patients. Methods: 70 LDH patients were selected in this part of study according to the quantii ed indexes for the clinical manifestation of the patients, such as visual analogue scale(VAS), Japanese orthopedic association scale(JOA), and straight leg raising test(SLR) before and after FSM therapy. The symmetrical indexes of spinal column(SISC) were detected by projection grating proi lometry image. Statistic correlation study was conducted between the symmetrical indexes of spinal column(SISC) and VAS, JOA, and SLR before and after FSM therapy by SAS 9.0. Results: There is significant positive correlation between the quantii ed indexes of clinical manifestation and the symmetrical indexes of spinal column(SISC) before and after the treatment. The coei cient correlation is signii cant between ratio of the ranges of motion between the left and right lateral l exion(LR), Lateral curvature angle(M) and quantii ed indexes for the clinical manifestation of the patients. The weight factors of the ratio of the ranges of motion between the left and right lateral l exion(LR) and hump angle(HA) are biggest. Conclusion: It is signii cant correlation between the quantii ed indexes of clinical manifestation and the symmetrical indexes of spinal column(SISC), such as LR, M, HA, before and after the treatment. The study proves that the symmetrical indexes of spinal column(SISC) can objectively elucidate the spinal function and state of patient recovered. The mechanics balance of spinal column must be thought highly of in the manipulative treatment of lumbar disc herniation(LDH) patients.更多还原
